- Monitor clients' progress and document observations in accordance with standard procedures.
- Participate in the admission, case planning, and discharge process for clients by conducting interviews, gathering relevant records, and attending case meetings.
- Establish rapport with clients and provide emotional support, counseling, and assistance with daily living activities.
- Assist clients with time management, organization, and interpersonal communication, and escort them to medical and social appointments.
- Participate in case planning, interview clients to identify problems and needs, and coordinate services with medical professionals and community resources.
What you bring
We are looking for a qualified and compassionate individual with a strong background in social services and a passion for supporting individuals with mental health and addiction challenges.
- Diploma from a recognized Social Services Support Worker educational program.
- Two years' recent, related experience, or an equivalent combination of education, training, and experience.
- Valid B.C. driver's license.
Skills and Knowledge
- Knowledge of the principles of recovery in mental illness.
- Knowledge of community, social, and health resources.
- Knowledge of substance misuse and of problems encountered by chemically dependent individuals.
- Knowledge of mental health issues and treatment.
- Ability to demonstrate and assist clients with development of health life skills.
- Ability to establish and maintain rapport with clients.
- Ability to observe and recognize changes in clients.
- Commitment to developing knowledge and understanding of legislative obligations and provincial commitments related to Indigenous reconciliation.
