We are seeking a compassionate and skilled Community Health Worker to join our team at Elizz, a leading healthcare provider in Canada. As a Community Health Worker, you will play a vital role in providing home support services to clients, promoting their physical and emotional independence.
Key Responsibilities- Provide home support services to clients, including assisting with activities of daily living, delegated tasks, and light housekeeping.
- Develop and maintain effective relationships with clients, promoting their physical and emotional well-being.
- Communicate changes in client condition to supervisors and maintain effective relationships with other team members.
- Proactively identify and promote client safety, seeking support from supervisors as needed.
- Community Health Worker (CHW) or Health Care Aide (HCA) certification.
- Experience in healthcare, preferably in a community setting.
- Current First Aid (Standard) / CPR Certificate (Level C).
- A valid driver's license and vehicle.
- Current immunization record, including a TB test completed within the past 12 months.
- Current vulnerable sector check screening.
- Availability to work days or evenings, including weekends.
- Excellent organizational and interpersonal skills, with the ability to work independently and as part of a team.
