Communicable Disease Outreach RN position at Saskatoon Tribal Council, working with First Nations communities to bridge health services and provide education, testing, and case management for HIV, Hep C, and STBBIs.
Job Description
The Communicable Disease Outreach RN will work to bridge health services on and off reserve for people living at risk or who have contracted HIV, Hep C, or other STBBIs. The role requires a strong background in sexual health, excellent interpersonal skills, and experience in working with First Nations communities.
**Key Responsibilities:**
* Assist and support health services in member communities and at the STC Health Centre, focusing on HIV, Hepatitis C, STBBIs, sexual health, and harm reduction.
* Collaborate with the STBBI team to provide harm reduction education for staff and clients in urban and rural communities.
* Provide assistance and support to communities' harm reduction and sexual health programs.
* Engage with urban and rural, indigenous and non-Indigenous communities to improve 90/90/90.
* Assist with case management, testing, and linkages to care and treatment.
* Collaborate with health professionals to evaluate clients' medical or physical condition and assess client needs.
* Support the review and development of HIV Nurse orientation packages.
* Make recommendations for program-related policies and procedures.
* Monitor the effectiveness of programs and services and make recommendations for improvement.
* Conduct community forums and participate in public education activities.
**Requirements:**
* Bachelor of Science in Nursing
* Minimum of 2 years of nursing experience
* Clinical experience in administering point of care testing, venipuncture, and immunizations is preferred.
* Experience in Community Health Nursing is an asset.
* Knowledge in the areas of HIV, HCV, STIs, and harm reduction.
* Demonstrated case management practices and experience in conducting case reviews are essential.
* Demonstrated knowledge and understanding of multi-cultural issues, addictions, mental health issues, family violence, sexual health, blood-borne infections, and issues related to systemic challenges.
**Working Conditions:**
* Moderate risk of exposure to disease or illness through client interaction.
* Exposure to clients who may have impaired judgment due to emotional distress, mental illness, or intoxication.
* Willingness to work flexible hours, including evenings and weekends.
* Travel within STC communities and assigned areas of the program.
* Valid Saskatchewan driver's license, reliable vehicle, and meeting STC's insurance requirements.
* Current, original Canadian Criminal Record Check (CPIC) and vulnerable sector check as a condition of employment.
