Responsibilities- Provide holistic, evidence-based care to patients with various heart conditions, acute and chronic kidney conditions, and heart arrhythmias.
- Develop and implement care plans, integrating nursing activities with other inter-professional activities.
- Collaborate with the care team to coordinate patient care activities and ensure seamless transitions.

- Participate in quality improvement initiatives and evidence-based practice implementation.
- Current registration with the College of Nurses of Ontario.
- Health Assessment Course or equivalent required.
- Current BLS/CPR certificate required.
- Coronary Care I or equivalent required.
- One year recent clinical experience with cardiac and respiratory patients, and with telemetry, preferred.
- Demonstrated understanding and champion of patient and family-centered care.
- Demonstrated history of initiating and supporting organizational change and improvement initiatives.
- Demonstrated commitment to ongoing professional development and learning.
- Strong evidence of an interprofessional and collaborative approach to patient care.
- Excellent communication and interpersonal skills and the ability to function as a team player.
- Critical thinking/problem solving skills.
- Strong patient teaching skills.
- Satisfactory attendance record.
- COVID-19 vaccination or documentation required.
- Ability to work in a fast-paced environment and adapt to changing situations.
- Strong leadership and collaboration skills.
- Excellent communication and interpersonal skills.
