Home & Community Care Nurse (HCCN)

Seeking a skilled Home & Community Care Nurse to provide nursing services to elderly, chronically ill, and disabled clients in their homes, promoting independence and health promotion.

About the Role

The Home & Community Care Nurse will be responsible for assessing client needs, developing care plans, and providing nursing and support services to enable clients to stay in their homes and promote activities of daily living (ADLs).

Key Responsibilities
  • Ensure total confidentiality is maintained for all client files, records, and correspondence
  • Ensure the security and monitoring of drugs used for home community care nursing program
  • Evaluate program results and effectiveness annually through feedback from clients, statistical data, client records, and a review of the home community care nursing component of the CHS plan
  • Delivers approved health care services through planned home visits, clinics, and workshops within scope of practice
  • Redirects to appropriate client care referrals, internal and external
  • Creates and reviews care plans, through individual assessments
  • Sets priorities for client care for individuals
  • Immediately and effectively report changes in relation to care plans and duties to Care Aides
  • Clinical oversight of the Home Care LPN and Care Aids, including delegation of tasks and providing care-plan updates and education
  • Participate in team meetings with the Home Care Aids and the Support Workers
  • Informs team members of with relevant up-to-date education and procedures and standards
  • Supports arrangements for palliative care, Visits palliative care clients.
  • Ensures clients have access to support programs such as footcare, bathing program, diabetes and immunization supports
  • Ensures all equipment related to client care is hygienic through proper sanitation
  • Coordinates blood work with on site providers
  • Liaise with care providers as each client requires including physicians, Interior Health, physiotherapists, patient care and dieticians
  • Arranges and participates in joint home visits to families with other professionals
  • Participate in case conferences
  • Provides wound care when necessary
  • Demonstrates care techniques, and methods of disease prevention and rehabilitation
  • Encourages healthy lifestyles by assisting other staff with health promotion activities
  • Assists clients to identify their own health needs and take appropriate action to meet those needs
  • Collaborate with other nursing staff to implement the prevention, health promotion, and other programs identified in community health nursing planning
  • Assess community health needs using local knowledge and information. morbidity, mortality, and general health status statistical information
  • Monitors the health of the elderly through assessment and screening clinics and early intervention for identified health problems
  • Promotes the maintenance of a safe and healthy environment in homes and the community at large
Administrative Duties
  • Develop Home and Community Care work plans client care plans, communicate plans with LPN's and CCA's, providing education and training where appropriate
  • Collaboratively Develops promotional materials to communicate the purpose and accessibility of the Home Community Care program and its support services
  • Reporting as required by funder(s) and leadership
  • Conducts home/office visits
  • Attends professional development training to provide current care, techniques and practices while delivering Home Community Care Program
  • Attend community meetings and workshops that relate to the HCCN position
  • Promote traditional and cultural health practices.
  • Ensures that care is provided according to all relevant policies, procedures, and regulations.
  • Coordinates Case Management when necessary
  • Provide information to other health care professionals, as required
  • Order supplies for Home Community Care program keeping accurate inventory and records
  • Develop and maintain a current, accurate, confidential client reporting and filing system
  • Biohazard management
  • Attend and/or host meetings internally and externally, as required
  • Make recommendations for changes and improvements to the program, when opportunities are identified through incident, audits, or case scenarios
  • Reports any concerns to the Director of Health and Social Development
  • Performs all duties and responsibilities in accordance with the OKIB policies and procedures
  • Performs other duties as required
Requirements
  • Bachelor's degree in nursing (RN) from a recognized university
  • Current, active registration with BC College of Nursing Professionals (BCCNP)
  • Certified in wound care management
  • Training in diabetes and chronic disease management
  • Current First Aid/CPR certificate with AED
  • Valid BC Driver's licence, minimum Class 5 and reliable vehicle
  • Minimum two years experience in acute care or related community care nursing
  • Managing clients with Chronic Obstructive Pulmonary Disease
  • Experience working in a hospital setting and community setting
  • Experience working with First Nations communities
