About the Role

CBI Home Health is seeking a skilled Speech Language Pathologist to join our team in Toronto. As a Home and Community Speech Therapist, you will have the opportunity to work with a variety of clients and populations, providing comprehensive triage services, holistic speech, language, and swallowing assessments, and personalized interventions.

Responsibilities
  • Assessment and implementation of SLP interventions in a home-based setting
  • Use of a collaborative and interdisciplinary team-based approach to care and services
  • Support a holistic and restorative approach to care, including enhanced stroke pathway and cognitive communication supports
  • Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention
Requirements
  • Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  • Focus on person and family-centered care
  • Comfortable delivering care both in person and using virtual methods/technology
  • Passionate, empathetic, and able to apply critical thinking when making client decisions
  • Excellent communication skills and ability to work in collaboration within an interdisciplinary team
