Job Title: Licensed Practical Nurse, Hemodialysis Company: Fraser Health Job Description: The Licensed Practical Nurse, Hemodialysis will provide practical nursing services during the pre-dialysis, intra-dialysis, and post-dialysis periods in a hemodialysis environment. The successful candidate will work collaboratively with the interprofessional team to assess, plan, implement, and evaluate patient care, ensuring a high level of patient satisfaction and safety. Key Responsibilities: • Assess, plan, implement, and evaluate patient care during the pre-dialysis, intra-dialysis, and post-dialysis periods • Establish and maintain a therapeutic relationship with patients and their families • Contribute to the development and ongoing changes to individualized plans of care • Perform patient care duties in collaboration with the Registered Nurse and other members of the nephrology team • Plan, organize, and establish client care-plan priorities • Promote client involvement in health care decisions • Document and update information on relevant care and procedures • Administer medications and provide medication education to clients • Participate in quality improvement and program activities Qualifications: • Grade 12, plus graduation from an approved program of Practical Nursing • Successful completion of post-basic education through an in-house program offered by a health authority affiliated with BC Renal • One (1) year recent related experience working as a Licensed Practice Nurse in acute care • Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) • Current certificates in CPR Level C or equivalent and First Aid About Fraser Health: Fraser Health is a leading healthcare provider in British Columbia, serving a diverse population of over two million people.
