Outreach Support Worker, Indigenous

Provide outreach community-based support services to Indigenous clients struggling with homelessness and living with substance use/mental illness/chronic medical concerns.
About the JobAs an Outreach Support Worker, you will work with a culturally diverse population, providing individualized support to clients in their homes, communities, and other settings. You will assist clients with daily living tasks, provide emotional support, and connect them with community resources.
Key Responsibilities- Provide outreach support services to Indigenous clients, including daily living tasks, emotional support, and connecting them with community resources.
- Assist clients with accessing healthcare services, social services, and other community resources.
- Develop and implement individualized plans to support clients' goals and needs.
- Provide education and support to clients on healthy living, substance use, and mental health.
- Collaborate with other service providers to ensure a comprehensive approach to client care.
- Diploma in Community Social Services, e.g. Addictions Support Worker, Mental Health Worker, Community Health Worker, or Social Services Worker inclusive of counselling skills training.
- Two (2) years recent related experience working with individuals with mental illness/substance use issues, including teaching life skills, experience with Indigenous culture and practices, cultural safety and practices in Indigenous community services.
- Current CPR certificate. Current Food Safe certificate. Valid Class 5 BC Drivers Licence and access to personal vehicle with appropriate business insurance coverage.
