Job DescriptionThe University Health Network, a leading research teaching hospital, is seeking an experienced Nurse Practitioner to join the Jay and Sari Sonshine Centre for Stroke Prevention and Cerebrovascular Brain Health. As an Outpatient Nurse Practitioner in Lifestyle and Health Promotion, you will be responsible for promoting health and preventing illness in individuals, families, and groups, with a focus on stroke prevention and brain health.
Key Responsibilities- Conduct comprehensive assessments and diagnose and manage the care of patients with stroke or at risk for stroke.
- Plan and implement strategies to promote health and prevent illness, including hypertension and diabetes.
- Deliver, coordinate, and participate in educational and professional development activities/programs.
- Work with community agencies to plan and deliver group-based health promotion, disease prevention, and education activities.
- Develop and maintain effective working relationships with existing community and professional groups.
- Master's degree in Nursing or completion of a master's degree program.
- Registered Nurse (EC) certification complete or pending.
- Nurse Practitioner (NP) Adult or Primary Care certificate complete or pending.
- Over 10 years of experience in Primary and Community Care and Health Promotion.
- Experience in nursing research and quality improvement methodology.
