Key Responsibilities- Assess patients' nutritional needs and develop personalized treatment plans to optimize their health and well-being.
- Provide education and counseling to patients, their families, and caregivers on healthy eating habits, nutrition, and disease management.
- Collaborate with healthcare professionals to ensure comprehensive care and support for patients.
- Conduct thorough assessments of patients' nutritional status and develop effective treatment plans to address their specific needs.
- Develop and implement nutrition education programs for patients, their families, and caregivers.
- Stay up-to-date with the latest research and best practices in nutrition and dietetics to provide evidence-based care.
- Registered Dietitian in good standing with the College of Dietitians of Ontario (CDO).
- Relevant experience providing nutrition and dietetic services in a community or healthcare environment.
- Comprehensive knowledge of dietetic services, practice methods, professional skills, nutrition principles, and strategies.
- Strong interpersonal and collaboration skills to work effectively with patients, families, caregivers, and healthcare professionals.
- Good understanding of the roles of other healthcare professionals and complementary therapies.
- Knowledge of funding agencies available to assist patients in accessing needed support services and other community resources.
- Practical knowledge and understanding of pertinent legislation.
- Comprehensive knowledge of the standards of practice and professional guidelines set forth by CDO.
- Ability to prioritize professional duties and manage caseload in a time-efficient manner.
- Ability to accurately complete required documentation, reports, forms, and recommendations.
- Effective listening, observation, and facilitation skills.
- Ability to communicate information effectively through a variety of means, including meetings, reports, letters, and presentations.
- Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel).
- Must have valid driver's license and access to a vehicle.
- Advanced oral and written proficiency in English is essential.
- Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
- Experience with management of wound care, post-operative GI surgeries, motivational interviewing, diabetes, oncology, gut health, food insecurity, and chronic disease management.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.
