About the role

As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community. You’ll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work both independently and collaboratively as a member of a multidisciplinary team will allow you to influence your team’s success.

Core Responsibilities

  1. Ensure the supported persons’ rights and choices are respected and advocate as needed.
  2. Interact effectively with people we support.
  3. Empower the people we support to be full citizens of their community.
  4. Participate in the development, implementation, and documentation of Personal Service Plans.
  5. Foster positive relationships with persons’ family members, professionals, co-workers, and members of the community.
  6. Assist with activities of daily living and medication administration.
  7. Excellent understanding of behaviour management principles and strategies including mental health needs.
  8. Record the daily activity and personal health information (PHI) of the people we support on our case management database.
  9. Work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties.

You will have

  1. Grade 12 education.
  2. Community Support Worker Diploma or equivalent.
  3. Minimum 2 years of experience in community social services/health care/caregiver.
  4. Experience in Dysphagia.
  5. Experience in using mobility equipment and safety devices.
  6. Experience in Bipap Management, Oxygen Administration, Seizure Management, and Airway Management.
  7. Current Emergency First Aid/CPR and Food Safe certificate.
  8. Previous behaviour management course/training.
  9. Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
  10. Non-verbal communication skills.
  11. Valid Class 5 Driver's Licence.
  12. Satisfactory Driver’s Abstract.
  13. Criminal record clearance for children and vulnerable adults.
  14. Proof of COVID-19 vaccination status as per current Public Health Orders.
  15. TB Test.
  16. Moderate computer literacy.
  17. Eligibility to work in Canada.
