Reporting directly to the Chronic Disease Management (CDM) Team, and indirectly to the CDM Manager and Cardiovascular Lead, the Stroke Rehabilitation Therapist is responsible for the day-to-day leadership and coordination of stroke programs and services at the assigned site. The Stroke Rehabilitation Therapist is responsible for mentoring, responding to clinical questions and coordinating ongoing skill development to deliver effective clinical stroke services based on evidence and population needs across the continuum of health. Additionally, this role coordinates the day-to-day planning, development, delivery and evaluation of programs and services for the assigned area. This position typically leads interdisciplinary program staff to provide evidence-informed services within program parameters and in adherence with Alberta Health Services (AHS) and Canadian Stroke Best Practices. The Stroke Rehabilitation Therapist participates in and implements quality improvement initiatives, program development, evaluation and knowledge translation within the program. The Stroke Rehabilitation Therapist is a key participant in zone and provincial initiatives. Stroke Rehabilitation Therapist collaborates with discipline-specific Professional Practice Leads (PPLs) to implement supported and leading practices. Stroke Rehabilitation Therapists may consult on complex patient care scenarios and may provide clinical services.

As a Physiotherapist II, you will provide coordination of physiotherapy and other allied health clinical services. You will provide supervision of students and team members as assigned. You will work both independently and as part of an interdisciplinary team providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure physiotherapy practice is evidence based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be coordinating the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will participate in program development and operational planning and representing the department on committees or programs. You will make decisions on staffing level requirements, workflow, caseload priorities in collaboration with managers and other healthcare providers. AHS is dedicated to supporting our Physiotherapists in their practice area and expanding their skillsets regardless of location. All AHS Physiotherapists have access to a list of experienced clinicians across the province and are able to tap into this resource via email, phone and virtual meetings. As we transition to our Provincial Electronic Medical Record (Connect Care) you will also be able to share patient information real time with these senior clinicians to assist you in providing high quality care.

Master's degree in Physiotherapy from an accredited university program or bachelor's degree in Physiotherapy from an accredited university program and related experience. Active or eligible registration and practice permit with the College of Physiotherapists of Alberta.

Registration with the appropriate Alberta regulatory college or professional association. BLS. NIHSS, Swallowing screen training. A minimum of 3 years of clinical experience in a related area is required.

Experience in stroke rehabilitation is an asset. Educational and/or other qualifications required to perform in the clinical area. A demonstrated clear pattern of professional and personal development.
