This hybrid role involves a combination of responsibilities in accordance with provincial and federal laws, standards, and regulations governing the practice of pharmacy, the Ministerial Order for Clinical Pharmacy Services, site policies, and internal Mint Health + Drugs policies and procedures.

Dispensary Fulfillment Operations:

The pharmacist will manage the execution and distribution of medications for contracted homes, detox facilities, emergency shelters, and other programs we service.

Day-to-Day Responsibilities:

  • Triaging and processing all medication orders for the pharmacy.
  • Overseeing pharmacy operations to ensure efficiency and optimal patient care.
  • Accurately and promptly packaging and dispensing prescriptions.
  • Consulting and providing advice to key stakeholders, including managers, physicians, and nurses.
  • Performing other duties as assigned to support Mint Health + Drugs: CMP.
  • Training and mentoring pharmacy team members as needed.

The pharmacist will ensure continuity of care for clients through collaboration with onsite physicians, nursing staff, and other health providers at long-term care, supportive living, and other congregate living communities.

Day-to-Day Responsibilities:

  • Providing clinical pharmacy services to patients in alignment with Continuing Care Service Standards, site policies, internal policies, and the Ministerial Order of Clinical Pharmacy Services.
  • Collaborating with Mint Homecare nurses to maximize patient health outcomes.
  • Conducting annual and quarterly medication reviews, hospital discharge medication reconciliations, etc.
  • Establishing collaborative practice networks with physicians, nursing staff, health care aides, case managers, and other facility staff.
  • Providing educational in-services as required.
  • Adhering to internal policies and processes established by the Clinical Division.
  • Establishing and maintaining effective working relationships with physicians, other health practitioners, patients, and family members.
  • Supporting operational activities through coordinated service delivery, facilitating site-to-pharmacy communications, and ensuring continuity of care for patients.
  • Performing other duties within their scope of practice as outlined in the job description.
  • Completing other duties as assigned.

Education and Experience:

  • Bachelor’s degree in pharmacy and valid licensure.
  • Additional Prescribing Authority or willingness to attain.
  • Valid Injection Certification or willingness to attain.

Qualifications and Skills:

  • Proficiency in pharmacy software and database programs.
  • Strong interpersonal, communication, and customer service skills.
  • Excellent organizational and time management skills.
  • Takes initiative and exercises independent judgment.
  • Remains current with advances in health and pharmacy-related matters.
