Indigenous Clinical Patient Navigator

JOB SUMMARY:
The Indigenous clinical patient navigator will act as a resource and point of contact for both Indigenous Clients and their families, healthcare providers and when appropriate, community members, to ensure the provision of care is culturally safe and client centered. In collaboration with patients and families, healthcare providers and community resources, the Indigenous clinical patient navigator will navigate the healthcare system by facilitating access to services and assist during discharge planning. Using a Two-Eyed seeing approach, Indigenous clinical patient navigator fosters an environment that supports traditional Indigenous ways of knowing and healing practices with evidenced based quality care. The goal of this position is to ensure Indigenous clients are accessing appropriate care from appropriate sources, increase client capacity and connections to be stable and supported.
REQUIREMENTS:
- Degree in a related health or social sciences discipline preferred or equivalent experience working in Indigenous health programming;
- Experience working in the healthcare system an asset;
- One to three years recent related experience working in Indigenous healthcare;
- Demonstrated knowledge of two-eyed seeing and Indigenous ways of knowing;
- Demonstrated understanding of Indigenous healing practice and person-centered care;
- Working knowledge of Indigenous Heath programming at the systems, community, provincial and federal level;
- Demonstrated knowledge of local Indigenous peoples’ demographics, languages, cultures, customs, and traditions;
- Demonstrated understanding of cultural safety and competency within healthcare;
- Demonstrated understanding of Canadian colonization, Indigenous systemic racism and its impact on the current health and wellness of Indigenous peoples
- Ability to foster trusting relationships;
- An ability to facilitate and/or make referrals as needed;
- Strong computer software skills (e.g.: PowerPoint, Excel, Word, etc.);
- Must be of indigenous descent and able to demonstrate their connection to their community and culture via a testimonial from their Band leadership (Chief and Council) or individual who is recognised by their community as a cultural leader (Elder, etc.);
- Ability to attend work on a regular basis;
- Ability to perform the duties of the position;
- Good work record;
- Written and spoken competence in English is required;
- Language/Mi'kmaq an asset;
- Lived experience;
- Ability to work independently and as a team member;
- Physically able to perform assigned duties;
- Good previous work history (performance and attendance);
- Adherence to professional ethics principles, to the Network management philosophy and organizational values;
- Compliance with the rules of confidentiality established by the Vitalité Health Network.
