Clinical Nurse Specialist, Geriatric Emergency Management - (12006)

Sinai Health is looking for a Clinical Nurse Specialist (CNS) for the Geriatric Emergency Management (GEM) role. The role is located at our Mount Sinai Hospital campus.

Mount Sinai Hospital is part of a network of senior-friendly organizations supported by The Regional Geriatric Program of Toronto (RGP). The GEM role supports health care providers in the delivery of interdisciplinary, senior-friendly, and evidence-based care that optimizes the function and independence of seniors and supports aging in the community.

In this role you will:

  • Provide assessments to identify acute symptoms, underlying health conditions, physical, functional, emotional and cognitive status, home environment issues and home supports needed for the at-risk elderly identified in the Emergency Department.
  • Make recommendations for care for frail seniors and assist in their implementation within the ED, and other hospital services, including specialized geriatric services.
  • Initiate and/or coordinate the implementation of a comprehensive plan of care developed in collaboration with the ED team, the patient and family, and other multidisciplinary team members as necessary.
  • Assist in the development of linkages with community services (e.g. family physician, Community Care Access Centre, long-term care setting, ambulatory care RGP services) and specialized geriatric services.
  • Educate patients/families/staff both formally and informally regarding the particular needs of the elderly and the specialized care they require in the Emergency Department.
  • Develop innovative solutions to address opportunities for improved transitions of care between the ED and other community providers with a view to building capacity in the system.
  • Provide support to the ED in developing an elder-friendly emergency health care environment.
  • Participate in performance and statistical reporting requirements.
  • Participate in Emergency department improvement and hospital-wide initiatives to provide and improve the care of seniors.
  • Participate in the RGP's GEM liaison group activities and maintain links with other GEM nurses.
  • Engage in other system initiatives in relation to Mount Sinai Hospital.
  • Participate with the ED Leadership team in related administrative department activities.
  • Supports and fosters the philosophy, goals and standards of the Department of Nursing.

Job Requirements:

Mandatory:

  • Master's Degree in Nursing required from an accredited educational institution.
  • Current certificate of registration with the College of Nurses of Ontario (CNO).
  • Minimum 3 years of recent experience in Emergency nursing or Acute Medical/Surgical nursing.
  • Demonstrated expertise in Geriatric Nursing.
  • Member of Registered Nurses' Association of Ontario (RNAO).

Preferred:

  • Canadian Nurses Association (CNA) certification in Geriatrics.

Skills and Knowledge:

  • Utilizes and demonstrates a comprehensive theoretical knowledge base and advanced level of clinical competence in the care of patients.
  • Effective verbal and written communication skills, interpersonal and customer services skills.
  • Proven experience successfully leading and facilitating change in collaboration with a multi-disciplinary team.
  • Excellent organizational and time management skills with the proven ability to manage competing demands.
  • Excellent decision making, problem recognition, problem-solving skills, negotiation and conflict resolution skills.
  • Commitment to collaborative practice.
  • Ability to work independently and efficiently in a fast-paced, multitasking, dynamic environment.
  • Ability to work effectively under stressful conditions.
  • Ability to utilize information technology.
  • Demonstrated satisfactory work performance and attendance history.
