West Park Healthcare Centre helps patients get their lives back by through the provision of specialized rehabilitation and complex continuing care services and a continuum of inpatient, outpatient and outreach care and treatment. West Park's 27-acre site is being transformed into an integrated campus of care, including a new hospital and additional community based services focused on seniors and people living with disabilities.

Job Overview:

The Nurse Practitioner would provide support to the Complex Continuing Care (CCC) program which includes a total of 52 beds. The Complex Continuing Care Service is a specialized 24-hour, nursing care program for people with complex or progressive illnesses or disease. Patient care is managed collaboratively by the Nurse Practitioner and Physicians as well as an interprofessional team that includes RNs, RPNs, Physiotherapists, Occupational Therapists, Respiratory Therapists, Social Workers, Psychologists, Psychiatrists, Dieticians, Recreation Therapists, Rehabilitation Assistants, Client Care Attendants, and others. The Nurse Practitioner would be joining a Professional Practice team where NP support and mentorship would be available.

The NP contributes to quality patient care, and improvements in organization and system outcomes. The NP demonstrates advanced knowledge, skill and judgment in providing care to patients that is consistent with the College of Nurses of Ontario's Practice Standards for Nurse Practitioners (2001) and the Canadian Nurses Association Competencies for Nurse Practitioners (2010). The NP works autonomously and collaboratively with physicians and the interprofessional team to provide clinical care to patients and to develop, implement and evaluate evidence informed practices in specific patient populations at West Park Healthcare Centre.

Accountabilities:

Clinical Practice
The provision of clinical care to patients comprises the central aspect of the NP role.
The NP works autonomously to perform comprehensive health assessment, performs a complete or focused health history , performs a complete or focused physical examination, identifying and interpreting normal and abnormal findings, synthesizes health assessment information using critical inquiry and clinical reasoning to diagnose health/illness conditions, formulates differential diagnoses through the integration of patient information and evidenced informed practice and treats and manages acute and chronic illness within a holistic model of care.
The NP orders and interprets screening and diagnostic tests, prescribe medications and performs procedures within the NP legislated scope of practice and the Centre's policies within defined patient populations including admitting and discharging patients as well as transferring patients to acute care.
NP is accountable to communicate with patients about health assessment findings and diagnoses, further required testing and referral to other health-care professionals. The NP is also responsible for patient follow-up. The NP counsels patients on symptom management, health maintenance, pharmacotherapy, alternative therapies, rehabilitation strategies and other health programs.
The NP makes appropriate referrals to other health care professionals within West Park as well as external health care providers.

The NP works in collaboration with physicians and the interprofessional team to provide consistent quality of care to vulnerable and medically complex demographic of mechanically ventilated patients to improve quality of life and reduce unnecessary emergency department transfers and intensive care unit (ICU) admissions. The NP also works in collaboration with acute care hospitals, the LHIN and community to assist in the transition of medically stable mechanically ventilated patient's home through the West Park Transition to Home Ventilation (THV) Program.

The NP works collaboratively with physicians, the interprofessional team, innovation and research and administrative partners to meet key responsibilities within the following domains:

1. Leadership: The NP provides leadership by:
Working in a manner that exemplifies the core values of the Centre: excellence, respect, trust, collaboration and accountability.
Acting as a preceptor, resource, consultant and coach to nursing colleagues, members of the interprofessional team and students.
Guiding, initiating and providing leadership in the development and implementation of policies, standards, practice guidelines, quality assurance, education and research initiatives, both corporately and program/unit specific.
Providing consultation to both program and corporate initiatives through committee involvement, and program development and evaluation.
Advocating for and participating in creating a practice environment that supports safe patient care, collaborative practice and professional growth.
Maintaining eligibility to be cross appointed to an academic institution and belonging to the Nurse Practitioner Association of Ontario and other relevant organizations

2. Knowledge Translation: The NP contributes to disseminating and embedding of evidence informed practice by:
Integrating evidence into clinical practice
Analyzing, apprising and synthesizing research and evidence informed practices.
Embedding research-based knowledge into daily practice and to support and educate other members of the health care team, including students.
Contributing to the development of patient/family education
Participating in the development and delivery of educational approaches for both patients and practitioners, including students.
Contributing to the dissemination of knowledge through presentations and publications.

3. Scholarship: The NP is a collaborative, innovative practitioner who supports scholarship by:
Collaborating with nurses, physician colleagues and the interprofessional team to evaluate outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends of specific patient populations.
Identifying issues related to clinical practice and patient care and recommending innovations to improve care approaches and care delivery
Recommending and being involved in quality improvement opportunities to promote safe, timely, effective, efficient, equitable patient-centered care.
Participating in research that contributes to improved outcomes for patients
Engaging in continuous professional development that demonstrates responsibility for maintaining and further developing NP competence.

Ability to work in a manner that exemplifies the Centre's core values and helps patients get their life back.

Qualifications:
Master's degree in nursing
Undergraduate Bachelors of Science in Nursing
Comprehensive knowledge of clinical health issues associated with specific clinical population(s) preferred
Demonstrated ability in best practices and/or protocol development, implementation and evaluation
Nurse Practitioner - Adult (NP-Adult), or
Nurse Practitioner - Primary Health Care (NP-PHC)
Current registration with the CNO as an RN (EC) in good standing
Certifications that complement clinical focus considered an asset Current/previous experience as an NP
Current experience and demonstrated success as a clinical nurse (3-5 years' experience)
Demonstrated ability to provide clinical leadership and mentoring to staff
Specialized clinical practice that supports patient populations at the Centre
Experience in research and quality improvement methodologies
