Unit Summary

The Community and Population Health Services (CPHS) portfolio supports the advancement of integrated health and social services from hospital to community. The Program is responsible for developing innovative partnerships with peer, community, and academic partners, providing expert leadership in pre- and post-hospital transitions.

CPHS is responsible for HHS' Population Health, Bundled Care, and Alternate Level of Care strategies. The portfolio offers system-wide support through Site Administrative Managers, Managers of Transitions, Transitional Care & Discharge Specialists, ALC Navigators, Centralized Care and Transition Team, Hospital to Home (H2H), Integrated Comprehensive Care (ICC) Program, Medical Psychiatry Consultation Liaison Team, Substance Use Services, and in collaboration with HNHB Home & Community Care Support Services.

Position Summary

The Nurse Practitioner (NP) in the Community & Population Health Program is registered with the College of Nurses of Ontario in the Extended Class - Adult and operates within the scope of practice of the RN-EC.

As a member of the Centralized Care and Transition Team (CCaTT), the NP collaborates with an interdisciplinary team, the Emergency Department team, and other team members using a patient and family-centered approach. Additionally, the CCaTT NP participates in the Virtual Nursing Care Program supporting COPD and HF patients following telepractice standards of the College of Nurses of Ontario and HHS policies. CCaTT NPs provide direct patient care, including assessment and treatment of patients with complex health needs, integrated care plan development for a smooth transition from hospital to community, and post-discharge follow-up.

At Hamilton Health Sciences, NPs mentor health professionals, engage in continuing education, lead programs, and contribute to the improvement of interdisciplinary care teams. The CCaTT NP collaborates with program leadership to enhance operational excellence and sustainability.

Schedule Work Hours

Monday to Friday with occasional weekends and holidays

Qualifications

  • Current Certificate of Registration in the College of Nurses of Ontario Extended Class - Adult
  • Completion of a Masters Degree in Nursing
  • Minimum of 5 years recent clinical experience working with older adults with complex health needs
  • Demonstrated knowledge of the chronic disease management framework
  • Demonstrated knowledge of community resources for integrated discharge planning
  • Ability to critically appraise literature and implement best practices
  • Knowledge in quality improvement and leadership
  • Effective communication and presentation skills
  • Experience in implementing innovative initiatives using change management
  • Strong problem-solving, organization, and prioritization skills
  • Critical thinking and interpersonal skills
  • Flexibility and ability to adapt to changing clinical situations
  • Proficient in information technology

Proof of full COVID-19 vaccination is required for employment.

Proficiency in both English and French is an asset.
