Case Coordinator - After Hours

  • Monday - Friday: 5am-9am or 5pm-1am
  • Saturday and Sunday: 5am-9am, 9am-5pm, 5pm-9pm, 9pm-9am
  • Mornings/afternoons/evenings on weekends
  • Shifts will be a combination of the above hours

Qualifications

The primary qualifications for this position include:

  • 1 year experience as a Care Coordinator for Government contracts (preferred)
  • Proficiency in Customer Service and ensuring a positive customer experience
  • Ability to multitask in a fast-paced setting
  • Positive attitude towards colleagues, clients, patients, families, and care teams
  • Readiness to work diligently, implement innovative ideas, and strive for excellence

Major Duties

  • Coordinate Community Care + File Management
  • Organize Care for families & loved ones with our exceptional Community Care Professionals
  • Maintain regular communication with Field Staff and patients/families for a superior customer experience
  • Utilize cutting-edge scheduling technology in the Home Health Care Industry
  • Meet with the Home Care Manager daily for priority setting and progress updates
  • Weekly discussions with Division Manager for progress updates and alternative suggestions
  • Address client service needs in consultation with Home Support Supervisor; schedule employees accordingly based on client requirements and staff qualifications
  • Ensure consistency in caregiver assignments and service coordination
  • Monitor referrals and care assignments via advanced technology systems
  • Collaborate with other Case managers, funders, or government programs as necessary
  • Place clients' schedules in the system and maintain accurate scheduling information
  • Assist with the supervision, recruitment, and orientation of Field employees
  • Document all calls from field staff promptly and appropriately
  • Assist in training and mentoring new staff members
  • Follow up with clients post-shift to confirm compatibility
  • Handle and document client and staff concerns while maintaining confidentiality
  • Monitor technology platform for Care Pro efficiency and accuracy
  • Provide timely and courteous responses to customer inquiries and needs
