Detailed Overview:

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

Reporting to the Manager of the assigned team, the Coordinator, Regional Quality Review is responsible for organizing the intake, triage and processes surrounding patient safety event reviews, and coordinates, liaises, and provides guidance regarding quality review processes across the Fraser Health Region for managers within acute, residential and community related programs.

Collaborates with FH leadership and management teams including internal and external stakeholders to provide support for system reviews of critical incidents and provides related summary of facts to the respective leadership team.

Responsible for theming of critical review recommendations that outline regional needs for systemic improvement, and communicates these to senior leadership.

Promotes a culture of quality and patient safety across Fraser Health, using a patient-centric lens. Promotes patient and staff privacy and confidentiality requirements, and maintains a neutral, patient-centered perspective.

Responsibilities:

  • Acts as a resource for operational leaders pertaining to Regional Critical Incidents (CI) reporting process, notification process and SBAR (Situation-BackgroundAssessment-Recommendation) initiating process via teleconferencing or other means. Guides operational site teams in Site Directorled SBAR processes such as by introducing the SBAR process for the session and facilitating adherence to the SBAR process for all Level 4 (severe harm) & Level 5 (death) incidents.
  • Contributes to the development of a regional culture of patient safety excellence by supporting clinical site operations / clinical programs with completion of critical incident (CI) reviews by methods such as initiating and facilitating CI reviews, ensuring adherence to CI review processes, and acting as a consultant to guide teams in identification of contributing factors to patient harm as well as creation of effective actions to prevent further harm to drive programspecific change.
  • Assists and participates in the development and updating of standardized processes for Quality Reviews and Medical Management Reviews,such as by preparing guidelines, policies, procedures, tools and methods; maintains and facilitates usage of critical incident review and medical management review recommendations theming template; reviews related documents and provides input as appropriate in alignment with Site Leadership needs and best practices.
  • Develops and delivers training related to the goals of patient safety focused on Quality and Medical Management Review tools and resources, including familiarization with PSIM policies and procedures, steps for completion of processes, and creation of effective recommendations based upon the SMART model. Prepares QRT materials for use for presentations, workshops, FH site orientation, and QI training sessions for FH Leaders, employees, physicians and patient representatives. Actively participates in other regional educational and culture change activities within the Clinical Quality & Strategic Priorities portfolio.
  • Provides work direction to the Coordinator, Site tQI who supports the regional CI process such as by creating recommendation summary reports and troubleshooting queries from operational leadership.
  • Supports other members of the Clinical Quality & Strategic Priorities team by acting as an internal resource and collaborating regarding CI recommendations, including by proactively sharing current and updated information regarding patient safety incident management policies and procedures to promote and ensure knowledge transfer to all FH leaders and employees.
  • Analyzes and assimilates complex information from CI reviews and local recommendations and organizes theming of recommendations to provide FH systemwide, evidencebased recommendations.
