The Lead-Palliative Care Implementation leads implementation of the Palliative Care Health services Delivery Framework ("The Delivery Framework"), initially focused on model of care recommendations for adults in community-based settings in their OH Region. They will act as a key connection to provincial level supports. They will oversee change initiatives to execute the strategic direction for palliative care, as outlined in the Delivery Framework. They will lead the phased operationalization of the Delivery Framework among implementing organizations across the OH Region. They are responsible for building relationships, planning, and monitoring implementation, coordinating palliative care education, quality improvement efforts and resources as well as informing evaluation activities. Where Indigenous communities wish to be involved, they will provide support to Indigenous communities and Indigenous organizations for their implementation efforts, in collaboration with regional partners.

This role is regional in nature and pertinent to the needs of Thunder Bay and surrounding area (Northwestern Ontario). Travel within the region or assigned geographic catchment area is required.

Here is what you will be doing:

  • Provides palliative care subject matter expertise to build a stronger regional palliative care system, through the implementation and operationalization of the Delivery Framework.
  • Responsible for multiple workstreams and deliverables of a high priority, complex initiative with multi-year duration. Directors multiple operational tasks concurrently.
  • Develops and actions work plans, applying provincial guidance within the region to deliver on accountabilities for local success.
  • Acts as a liaison with Ontario Health Region Managers, Directors, and senior leadership (as required) as well as with other key Ontario Health internal and external stakeholders, including the Ontario Health Teams.
  • Conducts regularly scheduled status reviews, provides business guidance, proactively identifies deviations from the work plan.
  • Leads and accelerates change in communities organizations by applying quality improvement methodologies to deliver best practice in palliative care in collaboration with identified clinical leadership.
  • Leads the spread and scale of implementation within and across settings.
  • Supports system and regional-level performance measurement and management, with an emphasis on using data for improvement and to inform planning.
  • Ensures timely completion of reports focused on implementation progress. Maintains documentation as required.
  • Leads engagement activities with provincial and regional stakeholders, through planned engagement sessions, including working groups, expert committees, advisory councils, and focus groups.
  • Develops and presents material for review and informational purposes using excellent written and verbal communications skills to express complex concepts effectively.
  • Works closely with regional and local clinical experts to organize palliative care education.
  • Works closely with regional and local clinical experts to organize palliative care coaching and mentoring.
  • Work closely with leaders of the community organizations, clinicians, and other stakeholders to identify opportunities to align clinical and operational priorities.

Here is what you will need to be successful:
Education and Experience

  • A Bachelor's degree is required, with a preference for a degree in health science, health administration, business administration or a related discipline.
  • A master's degree in one of the disciplines above is an asset or equivalent combination of education and experience.
  • A minimum of five (5) years of relative experience is required, including at least three (3) years working within the health care sector and leading/enabling strategic initiatives.
  • Experience with palliative care initiatives is required.
  • Certification in quality improvement (i.e., Lean, Sigma Six), project management (i.e., PMP), and/or change management is preferred.
  • Experience in change management, process development/improvement and/or project management is an asset.
  • A strong understanding of regional priorities and local health care needs within Ontario's health care landscape.
  • Experience with community health organizations or long-term care homes is an asset.
  • Fluency in French is an asset.

Knowledge and Skills

  • A commitment to developing a coordinated and standardized approach for delivering palliative care services all people in Ontario, regardless of their age or illness.
  • Knowledge of relevant legislation, including the Fixing Long-Term Care Homes Act, and the Excellent Care for all Act is required.
  • Superior ability to facilitate decision-making: to analyze vaguely defined problems, to evaluate alternatives, and to identify solutions.
  • Excellent critical and strategic thinking.
  • Able to prioritize, effectively anticipate and respond to issues as they arise.
  • Able to function independently is setting objectives for a team, leading team members, and managing expectations.
  • Strong relationship building skills, with demonstrated experience engaging and building relationships and consensus amongst diverse stakeholders.
  • Strong understanding of health care system and continuum of care including clinical processes with ability to use clinical data for improvement.
  • Excellent written and verbal communication skills.
  • Supports the advancement of equity, inclusion, and diversity across Ontario's health care system.
  • Commitment to engage with First Nations, Indigenous, Metis and urban Indigenous peoples as they self-determine their own approach to palliative care.
  • Advanced knowledge of Microsoft Outlook, Teams, Word, PowerPoint, and Excel.
