Job Summary:


As a Social Prescribing Seniors Community Connector, you will work with seniors who are referred from Health Care Professionals to identify what is affecting their health and wellbeing and support them to find services and activities that offer practical, social and emotional support that will improve their health and wellbeing.

You'll usually work with someone about six times, depending on their needs. They may have long-term conditions, mental health problems or feel lonely or isolated.


You'll build relationships with the people you are helping by listening carefully to what's important to them and what motivates them.

You'll work with them to create personalized support plans that gives them control over their health and wellbeing.

Your role will then be to connect them with local community and voluntary groups offering a range of activities and services.

This could be art classes, exercise groups or more practical supports such as financial and housing issues or food security needs.


Qualifications:

Education, Training and Experience:_

  • Postsecondary degree or diploma in the social sciences, health sciences or equivalent relevant education and experience.
  • Two (2) years of experience working with older adults, and their families, in a nonprofit, community or health care setting is preferred.
  • Strong knowledge of seniors' issues and challenges related to healthy aging.
  • Second language preferred.

Key Duties and Responsibilities:

Referrals_

  • Take referrals from a GPs and other members of the multidisciplinary health care team.
  • Manage and prioritize a caseload, in accordance with the needs, priorities and any urgent support required by seniors who are referred to the service.
  • Promote social prescribing, its role in selfmanagement, addressing health inequalities and the wider social determinants of health.
  • Take a proactive role in developing strong links with local health care providers to encourage referrals.

Wellness Plan development_

  • Meet participants on a onetoone basis, making home visits where appropriate and within agreed policies and procedures.
  • Build trust and respect with the participant, providing nonjudgmental and nondiscriminatory support, respecting diversity and lifestyle choices.
  • Work with individuals to cocreate a simple personalized wellness plan that will focus on improving health and wellbeing, introducing or reconnecting them to appropriate community groups and support services.
  • Seek advice and support from health care professionals connected with the participant, referring the patient back to primary care providers or other suitable health professionals if required.
  • Where appropriate, refer seniors back to health professionals/agencies, when what the person's needs are beyond the scope of social prescribing.
  • Identify local community assets and gaps in local provision.

Documentation and Evaluation_

  • Work sensitively with participants, their families and carers to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing.
  • Support the person making the referral to provide appropriate information about the person they are referring.
  • Provide appropriate feedback to the person making the referral as requested.
  • Document client interactions, wellness plans, and reports, and other administrative duties as required.
  • Seek regular feedback about the quality of service from participants and the impact of social prescribing.
  • Document and evaluate progress of work through regular reports to the funder and Director.

Professional Development_

  • Adhere to organisational policies and procedures, including confidentiality, working alone, information governance, equality and health and safety.
  • Work with your supervisor to access regular 'clinical supervision', to enable challenging issues presented by specific participants to be discussed and dealt with effectively.
  • Participates in collaboration with community agencies through networking and attending local Seniors' health task teams.
  • Participates in the Social Prescribing Program, Community of Practice, both province wide COP and Fraser Health Authority specific COP

  • Friday ( 9:00am
  • 4:30pm) 37.5hrs/week

Salary:
$22.00-$25.00 per hour
