We are currently seeking a Healthcare Case Coordinator to utilize their case management skills to address complex challenges in a fast-paced environment.

Success in this role hinges on effective case management, excellent communication abilities, empathy, thorough assessment and evaluation of member therapy, coordination with providers, and efficient time management for processing information for both new and existing members. The ideal candidate will showcase a strong work ethic, impeccable organization, focus, proactivity, and an ability to make an immediate positive difference for our members.

Job Description

Therapy Management

  • Take responsibility for overseeing new and existing therapy requests.
  • Thoroughly assess member treatment needs and collaborate with providers and specialty pharmacies to prioritize therapy requirements.
  • Maintain precise documentation of all interactions and therapy updates.
  • Continuously monitor and evaluate member therapy plans and progress.

Balance various responsibilities including case management, inbound and outbound contact, and adaptability to ensure seamless service delivery.

Member Advocacy

  • Understand each member's unique needs, preferences, and circumstances.
  • Advocate for members in therapy decision-making processes.
  • Assist members in overcoming barriers and provide information on medication options.
  • Empower members to actively participate in their treatment planning.

Case Resolution and Problem-Solving

  • Resolve member cases and inquiries efficiently, demonstrating strong critical thinking skills.
  • Review and address advanced questions or concerns from members, providers, and pharmacies.
  • Anticipate challenges and adapt approaches to meet evolving member needs effectively.

Communication and Collaboration

Proactively communicate member progress and therapy options to members in crises, ensuring personalized communication.

Engage in empathetic communication with providers to foster positive relationships and maintain high member satisfaction levels.

Collaborate with other departments to coordinate resources and address member inquiries effectively.
