Consumption and Treatment Services (CTS) is a health service that provides a safe and supportive environment for people to consume pre-obtained substances under medical supervision.

In addition to supervised consumption, individuals are provided with sterile harm reduction supplies, education on safer substance use/overdose prevention, medical monitoring post-consumption, medical assessment, counselling, and connections to primary care, addiction treatment, mental health support and other services.


The Consumption and Treatment Services Support Coordinator has a university degree or college diploma in a social work or other relevant discipline, or an appropriate combination of education and experience. An addiction counsellor certification is an asset. You have a strong knowledge of harm reduction philosophies and practices and have a passion for providing care to individuals using substances. You have your Emergency First Aid and CPR Level C (Health Care Provider level, preferred). You have excellent interpersonal and communication skills and a strong ability to work with and build positive rapport with marginalized individuals including those who are street involved, actively using substances, and who struggle with mental health issues. You are able to work independently with mínimal direction, work under pressure, problem-solve, take initiative and meet the needs of the client. You understand Indigenous Cultural Safety principles and trauma-informed care are able to problem solve, multitask and take initiative. You understand and are committed to the Mission, Vision and Values of the Guelph CHC.

PRINCIPAL JOB DUTIES

  • Deliver outreach services in the building (aka "Inreach"), including support, deescalation, crisis intervention, engagement, addiction counselling, and referrals.
  • Respond to concerns identified by staff, volunteers and building tenants by providing support and assisting clients to be connected to the services they require.
  • Build positive relationships with marginalized and vulnerable individuals frequenting the GCHC
  • Ensure GCHC is a welcoming space for all by clearly communicating GCHC client rights and responsibilities and asserting clear boundaries when necessary.
  • Ensure connections are made to needed services for CTS service users, including addiction treatment, primary care, mental health support, as well as other community resources.
  • Provide accompaniment and advocacy to ensure clients are able to access the care they need.
  • Work with clients and team members to create and communicate care plans
  • Liaise with other care providers in the system to ensure a seamless care experience for clients (i.e. Hospital, primary care, outreach services)
  • Provide flexible, stage matched, clientcentred counselling and support
  • Use a complex capable, trauma informed, harm reduction philosophy to deliver stage matched interventions
  • Provide support and assistance to Community Health Connect volunteers.
  • Assist with completion of system wide assessment tools including the GAIN and VI-SPDAT
