The Opportunity:


YSM is seeking a full time
Care Management Specialist who will be responsible to work with vulnerable youth that are experiencing poverty and facing barriers that threaten stability and employability.

Our goal is to support individuals in their journey towards stability, purpose and employment.

The Care Management Specialist role reports to the Evergreen Care Management Supervisor.

Responsibilities:

  • Utilize YSM's RISE Model (Respond, Invite, Support, Engage) to provide a wraparound system of support through internal and external partnerships to ensure that youth move from crisis to stability and out of poverty
  • Establish and strengthen relationships with external agencies, creating a pathway for new clients to enter YSM Care Management
  • Embrace and demonstrate a strengths perspective and trauma informed approach.
  • Support external system navigation: helping community members get access to services, including temporary housing, medical care and legal aid.
  • Helping to support youth as they transition out of youth services into the adult system
  • Work collaboratively with the workforce development team to connect youth with meaningful employment opportunities.
  • Engage in positive interactions and relationship building with key community stakeholders
  • Attend community and network meetings to gather relevant information pertaining to youth trends and programming in the community
  • Manage a caseload of up to 20 youth
  • Develop and deliver courses and workshops that are relevant to building capacity for youth to be successful
  • Document activities and outcomes through case notes, action plans, touchpoints using our TIMES measurement system

Qualifications:

  • BSW or equivalent certifications are an asset, but not a requirement
  • Able to work with diverse people groups within a multifaith, multicultural, antioppressive framework
  • Able to make analytical decisions around case management and program delivery
  • A demonstrated passion for poverty alleviation
  • Excellent written and oral communication skills
  • Capable of traveling around the city to assist with appointments
  • Strong attention to detail, administrative and computer skills
  • Experience preparing, delivering and facilitating workshops
  • Solid team player and able to work independently
  • Demonstrated personal commitment to YSM's Mission, Biblical Foundations, and Core Values

Additional:

  • A police check will be required.
  • First Aid & CPI training is an asset
