What you'll do

  • Assess patient and family needs for practical supports and assist families in accessing and securing resources internal and external to the hospital.
  • Provide support with education to patients and families related to their medical and/or psychosocial needs.
  • Provide community resource information and ongoing support services to patients, families and health care professionals, and facilitate referrals to community resources.
  • Assist the program staff with managing and communicating information on community resources, and creating systems to update and communicate this information to patients and families.
  • Participate in quality improvement activities including reporting unsafe situations and reports as per provincial and organization's policies and procedures.
  • Participate in the planning and implementation of special events and in the organization of activities for patients and families as needed.
  • Support research activities within the program as necessary.
  • Participate in team meetings and in other meetings relevant to the program.
  • Perform other related duties as assigned.

What you bring

Qualifications:

  • A level of education, training, and experience equivalent to a Bachelor's degree in an allied health discipline
  • Minimum of one (1) year experience in a health care setting working with a wide range of service providers.
Skills & Knowledge

  • Knowledge and experience in family support and pediatrics.
  • Extensive knowledge of community agencies and resources.
  • Ability to liaise effectively with staff at all levels of the organization and with partners in other health centers and community agencies.
  • Broad knowledge of Trauma Informed Care including Collaborative Problem Solving.
  • Excellent interpersonal, oral and written communication skills including training in reflective listening and validation.
  • Ability to work independently with limited supervision.
  • Selfdirected and highly motivated with excellent organizational skills.
  • Demonstrated sensitivity, confidentiality, and commitment to familycentered care.
  • Ability to collaborate effectively within a multidisciplinary care team.
  • Knowledge of Indigenous cultural safety, to provide health care services that are equitable, respectful and safe.
  • Ability to operate related equipment including word processing, spreadsheet and database programs at an intermediate level.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act
