What you'll do

Provide inclusive, pro-choice, non-judgmental care to patients and their partners
Experience with patient intake, assessments, treatment planning and counselling
Works within a harm reduction model of care

Assess patient and family needs for practical supports and assist families in accessing and securing resources internal and external to the hospital.

Provide support with education to patients and families related to their medical and/or psychosocial needs.

Provide community resource information and ongoing support services to patients, families and health care professionals, and facilitates referrals to community resources.

Assist the program staff with managing and communicating information on community resources, and creating systems to update and communicate this information to patients and families.

Participate in quality improvement activities including reporting unsafe situations and reports as per provincial and organization's policies and procedures.

Participate in the planning and implementation of special events and in the organization of activities for patients and families as needed.

Support research activities within the program as necessary.
Participate in team meetings and in other meetings relevant to the program.
Perform other related duties as assigned.

What you bring

Qualifications:

A level of education, training, and experience equivalent to a Bachelor's degree in an allied health discipline
Minimum of one (1) year experience in a health care setting working with a wide range of service providers.

You will also have

Knowledge and experience in family support and pediatrics.
Extensive knowledge of community agencies and resources.

Ability to liaise effectively with staff at all levels of the organization and with partners in other health centers and community agencies.

Broad knowledge of Trauma Informed Care including Collaborative Problem Solving.
Excellent interpersonal, oral and written communication skills including training in reflective listening and validation.
Ability to work independently with limited supervision.
Self-directed and highly motivated with excellent organizational skills.
Demonstrated sensitivity, confidentiality, and commitment to family-centered care.
Ability to collaborate effectively within a multidisciplinary care team.
Knowledge of Indigenous cultural safety, to provide health care services that are equitable, respectful and safe.
Ability to operate related equipment including word processing, spreadsheet and database programs at an intermediate level.
Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act

Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism and accompanying reports (BC DRIPA, TRC, etc.).
