Personal Support Worker

JOB DESCRIPTION
- Assist clients with their activities of daily living (ADL) based on their individual needs.
Includes, but not limited to:
- Bathing/showering
- Hygiene skin, hair, oral care
- Feeding
- Dressing/grooming
- Passive range of motion exercises
- Walk and active living support recreational activities (puzzles, music, simple crafts, etc.)
- Toileting/incontinence support
- Transfers and lifts
- Light housekeeping
- Safety supervision (monitoring for fall risk, wandering, appliance use)
- Companionship support
- Respite
- Checking vitals
- Follow individualized care plans as provided by Afya-Bora Home Healthcare Services INC
- Documents all aspects of care delivery to the client file and reports significant changes to management
- Good communication skills
- Make personal connection with clients whenever possible and engage them in hobbies, personal interests e.g. knitting, reading, cooking, etc.
- Promote and encourage the client to be independent in their daily living
- Ensure PSW tasks are completed while upholding the client's dignity and privacy
- Ability to create therapeutic alliance with clients, family and provide best care based on empathy, warmth, honesty and congruency.
- Share information about the client only within the trusted care network and only for a relevant purpose
- Work with the client's schedule to support their comfort and choices
- To the extent possible involve the client (and their family/POA) in making decisions about their care strategy
