Summary of Duties


Reporting to the Director of Care (DOC), the Activation Specialist utilizes activities of daily living (ADLs) and mobility to maximize independence, mobility, promote an optimal lifestyle and enhance quality of life for Sisters such as, activation and recreation assessment, intervention, and evaluation to support and/or maintain Sisters therapeutic recreation.

Working in collaboration with the Activities Coordinator as an interprofessional team member, the Activation Specialist participates in the coordination of activities that support an activation care delivery model and that contribute to maximum functionality.

The Activation Specialist will practice in accordance with Providence Motherhouse's Vision, Mission, and Values, operational policies and procedures, and standards of practice as set out by Therapeutic Recreation Ontario (TRO).

Attends weekly Multidisciplinary meetings as required.

Responsibilities:

Assessment, Clinical Documentation and Communication

  • Identifies information to be collected during the activation assessment (personal health information records, or the interprofessional team);
  • Selects and administers appropriate assessment tools based on needs and utilizes appropriate interview and observational techniques to gather information to inform the care plan;
  • Documents assessment findings, progress and outcomes;
  • Ensures informed consent is obtained for assessment and treatment;
  • Communicates key information regarding assessment and interventions in a timely manner to team members;

Intervention Planning and Program Delivery

  • Develops an intervention plan based on the Sisters goals and assessment results for both oneonone interventions as well as group programs;
  • Contributes to the overall teambased care plan;
  • Follows the Leisure Ability Model or other relevant Therapeutic Recreation models;
  • Identifies measurable outcomes related to the Sisters goals and needs and encourages Sisters to participate in activation programs taking into account their strengths, abilities, and any contraindications reflected in their care plan
  • Collaborates with other team members the participation, observed needs, expressed goals or other pertinent information and care needs;
  • Identifies and accesses relevant resources (e.g., adaptive devices) to support activity goals;
  • Complies with policies as they relate to safety during activities or on outings, and other applicable policies and procedures when managing responsive patient behaviours to ensure the best possible outcomes (e.g., Utilizes principles of Nonviolent Crisis Intervention/Gentle Persuasive Approach);
  • Designs individual and group programs to meet activation needs within a particular service or program that promote mobility and functionality;
  • Collaborates with Sisters, and the interprofessional team when developing therapeutic activation programs of interest, utilizing appropriate resources to enhance therapeutic program delivery (e.g., volunteers, support networks);
  • Develops and conducts formal and informal activity evaluation with outcomes and recommendations;

Health and Safety

  • Works in compliance with the Health & Safety Act and its regulations in performing duties in a safe manner and follows all health and safety policies, procedures and legislation including, wearing the appropriate Personal Protective Equipment (PPE), as required
  • Participates in staff meetings, education and/or training including for health and safety as required by the employer

Education and Experience

  • Degree or Diploma in Recreation and Leisure Services with a focus in Therapeutic Recreation;
  • Eligible for Registration with Therapeutic Recreation Ontario or Certified Therapeutic Recreation Specialist, preferred;
  • Experience in an array of services to adults that include community and hospital settings preferred (e.g., rehabilitation, dementia, adult acquired disability); ability to incorporate concepts into therapeutic participation in recreation programs;
  • Current CPR and First Aid certification;
  • Vulnerable Sector Check, level 3. Copies will not be accepted.
  • Experience in Microsoft Suite;
  • Experience working in a complex and high demand work environment;

  • Demonstrated knowledge of the Leisure Ability Mode
