Duties
Assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plansImplementing an intervention plan with patients, family members, significant others, and theinter-professional team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environmentReassessing and modifying the intervention plan as requiredLiaising with appropriate community agenciesCompleting applications for Alternate Level of Care (ALC) facilities, establishing contact with members of theinter-professional team and community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and futureRendering services for the immediate benefit of the Hospital and the communityProviding clinical teaching to Social Work Students, Medical Students, Professional Colleagues at University Health Network, Faculties of Social Work and in the communityParticipating in relevant research activities, projects, etc.Performing activities required for the effective and efficient operation/maintenance of the Social Work professional groupCompletion of a Masters Degree in Social WorkRegistration with the Ontario College of Social Workers and Social Service WorkersMembership in the Ontario Association of Social Workers, preferredThree (3) years experience working with an inter-professionalteam in a hospital setting, preferredExperience in acute care, preferredExcellent verbal and written communications skillsExcellent interpersonal, time management and organizational skillsExcellent problem solving skillsResearch experience/education an assetKnowledge of policies, legislation, program's and issues related to social welfare institutions and servicesKnowledge of a second language an assetIn addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks.
