As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do:

This position will provide float coverage for all patient services programs. The position may involve travel to all branches and hospital sites throughout the Central East region.


This Care Coordinator will be responsible for assessing patient eligibility for services/long term care placement, assessing patient care needs, and planning and authorizing service delivery to patients that best meet the needs of the patient within the mission, vision and values of Home and Community Care Support Services Central East.

This Care Coordinator will work independently and within a team to maximize quality outcomes for patients, as well as the organization.


What must you have?

  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers (RSW)
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problemsolving and decisionmaking skills
  • Effective time management, prioritization and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • Flexibility in adjusting to various work hours according to need.
  • A valid driver's license and a reliable vehicle are required, with the ability to drive to various locations dependent on patient visit.
  • Proficient in a Windows environment
  • Ability to complete 34 home visits daily or to travel throughout the unit/hospital to meet with patient/families and attend rounds, which may involve substantial driving or walking depending on the location of the visit, and includes patient assessments, forms completion (electronically and manually), and communicating with patients and families to ensure their understanding.
  • We have a mandatory COVID19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID19 vaccination status prior to start date.
  • Assumes other duties as appropriate.

What would give you the edge?

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language

Hours of Work
All hours of operation, including extended hours (70 hours per bi-weekly pay period).

Position Location and Travel
The position may involve travel throughout the Central East region. A valid Ontario driver's license and access to a reliable vehicle are required.
