The Masters of Social Worker (MSW) will be part of the Outpatient Mental Health (OPMH) team in the St Joseph's Mental Health & Addictions program.

The MSW, in conjunction with an interdisciplinary team, will provide comprehensive psychosocial assessments, individual therapy, case management and discharge planning in the Outpatient Mental Health Program (i.e. Concurrent Disorder Clinic, Collaborative Care Clinic, Rapid Access Crisis Clinic etc).The Clinician will complete intake for an adult mental health population, collecting collateral information from patients, physicians and community agencies to determine suitability for services.

The Clinician will develop and implement inter-professional plans of care to ensure appropriate and timely transitions between hospital-based programs and community care.

The Clinician will also participate in program development and quality improvement activities.

This position reports to the Manager, Outpatient Mental Health

Qualifications:

  • Master's Degree in Social Work field required.
  • Registration in good standing with OCSWSSW
  • Minimum 3 to 5 years¿ direct experience in acute hospital and/or outpatient mental health setting.
  • Experience in concurrent disorder strongly preferred
  • Sound knowledge of Ontario mental health legislation and DSM V classifications.
  • Proven ability to conduct indepth psychosocial mental health assessments, develop, implement and evaluate the interprofessional plan of client care for all clients.
  • Demonstrated ability to intervene in an appropriate and safe manner with individuals experiencing an acute mental health crisis.
  • Proficient knowledge of community resources and ability to facilitate transitions to internal and external services.
  • Experience providing brief psychotherapies and supportive counseling. Preference will be given to those with experience providing DBT and CBT.
  • Commitment to the principles of the Recovery Model and Trauma Informed Care.
  • Contribute to program development including quality improvement initiatives and supervision of students.
  • Advanced problem solving skills are required and ability to work independently.
  • The ability to work with clients of diverse cultural and social backgrounds is necessary.
  • Think critically, set priorities and organize work in response to client/patient needs.
  • Computer literacy is required.
  • Bilingualism (French/English) and/or proficiency in a second language would be an asset.
  • Able to function well under pressure and in a fluid environment.
  • Comfortable working independently and in an interdisciplinary team setting.
  • Comfortable working with a broad range of client groups and their families/supports
  • Satisfactory attendance record.
