Unity Health Toronto's St.

Michael's Hospital Mental Health and Addiction Service (MHAS) provides care to individuals with serious mental illness, substance use concerns and complex medical needs.

The Black TAY Program is a priority pathway for black youth to receive timely assessment by psychiatrist to develop detailed treatment plans and referrals to clinical appropriate resources and service such as SW support within the MHAS.

This role is in partnership with the Black Youth Helpline (BYH).

BYH is a not-for-profit organization that brings innovative, professional, culturally relevant perspectives to understanding and addressing the root causes of challenges faced by children, youth and families.

Its professional assessment process ensures that problems are first and foremost understood before they are addressed. Next steps are then driven by assessment outcome and the client¿s interest and goals.


The primary role of the
Social Worker (SW) is to provide individual (1:1) clinical work in MHAS to support the Black Transitional Aged Youth Program - youth between the ages of 16 and 29.

The SW will also be responsible for facilitating and coordinating evidence-based care that will include individual and group programing in the ambulatory setting.

This role will involve providing culturally relevant in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individuals referred from the Black Youth Helpline.

The SW will also provide counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education.- Work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care- Actively contribute to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community Identify and address educational needs for patients and their families- Demonstrate a commitment to ongoing professional development in order to continue to provide optimal patient care- Commit to, and participates in, ongoing quality improvement in the organization- Provide patient care in accordance with all professional and organizational standards- Demonstrate an understanding of the social determinants of and knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, Community Housing, CCAC etc.- Utilize active engagement outreach strategies to deliver services and supports in the client¿ home or in the community- Demonstrate excellent knowledge of recovery model and possess skills in crisis intervention and other evidence informed practice in therapeutic modalities (CBT, DBT, solution focused interventions, mindfulness).

- Utilize excellent problem-solving skills in establishing therapeutic relationships with hard-to-engage clients and support individuals in their goals

QUALIFICATIONS:

  • Must have a Master's Degree in Social Work (MSW)
  • Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas
  • Two (2) to Five (5) years of experience as a Social Worker
  • Direct experience partnering with and working closely with people belonging to communities that have been systemically marginalized and underserviced by health systems required;
  • Must have experience working with Black youth individuals who experience mental health and substance use concerns
  • Proven ability to provide culturallyinformed care for Black transitional aged youth population
  • Comfortable with community engagement and outreach
  • Eligible to engage in student education with academic partners
  • Current CPR certification
  • Excellent communication, critical thinking and interpersonal skills
  • Proven flexibility and adaptability in a fastpaced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care
