Position Summary


The Follow-Up Support Worker is a member of the EUTC Fresh Start team and will work together to provide services in a flexible, compassionate, and responsible manner to participants of the Fresh Start Project.

Support workers are to follow the Intensive Case Management model that utilizes the housing first philosophy based on; flexible and community-based support, promotion of stability, recovery and community integration.

The Follow-up Support Worker will receive direction, supervision and support from the Fresh Start Service Coordinator and the Program Manager of the EUTC.


Key Responsibilities

  • Collaborate with the participant in the formulation of a Service Plan that follows the housing first principles that is reviewed and modified with the participant on a regular basis.
  • Collaborate with internal project support including the Service Coordinator to develop an action plan to help participants to quickly move homeless individuals into stable permanent housing of their choice.
  • Collaborate with the Service Coordinator and external support available through the Doorways project (Winnipeg Rental Network) to communicate participant's housing needs and choice.
  • Provide outreach, case management, counseling, advocacy, and other needed services to participants in any environment including: the streets, shelters, prisons, hospitals, apartments, office, etc.
  • Provide individually tailored services to each participant—i.e. housing placement, independent living skills, vocational guidance, appointment escorts, integrated substance use treatment, family counseling, etc.
  • Assist participants to manage their money; prepare budgets with participants and compute expenses.
  • Educate, inform and advocate for participants regarding benefits and entitlements i.e. rights and responsibilities of tenant and landlord, employment and income assistance, employment insurance, food vouchers, etc.
  • Provide Life skills workshops and information sessions on various topics such as substance abuse, harm reduction, safe sex, domestic violence, employment, healthy living, wellness, etc.
  • Coordinate / monitor referrals to community services, and advocate participant participation in them.
  • Educate, inform and advocate for participants regarding benefits and entitlements.
  • Provide up to date information on community programs and services that the participant may be interested in partaking in i.e. employment programs, community sweats, etc.
  • Conduct monthly apartment inspections, document findings on inspection form.
  • Secure tenant keys for tenant move in; develop/operationalize master key plan for program site.
  • Ensure smoke detectors and carbon monoxide detectors are in working order in each apartment. Schedule cleaning and extermination of tenant apartments, as needed.
  • Maintain written and computerized records, compile reports and complete other program documentation (including case notes, statistics, letters, etc.) and perform other duties as directed.

Qualifications

Education & Experience

  • A minimum of an undergraduate degree or college diploma in the field of social service, or a combination of related education and training will be considered.
  • Demonstrated knowledge of Housing First.
  • Patience, creativity, flexibility, compassion, and sensitivity to persons who have experienced the Child Welfare and Criminal Justice System and trauma.
  • Skills in assessment, case management and advocacy.
  • Knowledge of Winnipeg services and resources in the areas of supportive housing, mental health, counseling, addictions, Child Welfare and Justice Systems and Aboriginal Culture.
  • Excellent interpersonal skills and ability to work cooperatively as a part of an interdisciplinary team of professionals.
  • Ability to communicate effectively, both orally and in writing.
  • Demonstrated ability to work independently with mínimal supervision.
  • Proficient in using Microsoft office
  • Word, Excel, Power Point and Outlook.
  • Adhere to AMC policies and procedures with specific attention to attendance, and professional ethics.
  • Must have a current satisfactory Criminal Record Check, and Clear Child Abuse Checks.
