Position Summary:


Under the general guidance of the Manager, and in collaboration with the Life Enrichment Staff (LES) Team the incumbent is responsible for the planning, coordination, and implementation of daily activities and programs for Adult Day Services (ADS) participants.

Additionally, the incumbent is assigned as the primary worker to support individual participants' successful experience within the program and in ensuring a client centered philosophy.


Responsibilities and Duties:

  • In conjunction with the Recreation Specialist, responsible for the planning and delivery of culturally sensitive, language specific activities for participants.
  • Lead, implement, and conduct activities based on participant's interests, capabilities and consistent with their care plans.
  • Actively seek information about participant's experience to adjust the daily plan for each, and to adjust the quality of activities and events.
  • Facilitate program to maximize participant engagement, provide opportunities for participation in programs/activities and provide leadership opportunities to participants.
  • Review new participant profiles and orientate the new participants to the program.
  • Develop full cycle programming plans including supplies/items required, for Manager review and approval.
  • In conjunction with the LES team:
  • Plan clusters to maximize participant involvement in the program/activity.
  • Monitor and facilitate participants' needs including changing needs are identified and effectively managed.
  • Identify additional service needs such as foot care or bathing services and notify the Client Care Counsellor (CCC).
  • Complete all required documentation including any changes in participant's condition.
  • Support and assist the CCC in identifying and addressing participant needs in a timely manner.
  • Review and implement care plans on a regular schedule.
  • Assist participants with feeding, toileting and personal care and act as backup to the Personal Support Staff as needed/ assigned.
  • Identify program supplies needed, ensure they are available and in good condition
  • Prepare and submit statistical and progress reports as required.
  • Ensure effective communication within the multidisciplinary service team and with participants and caregivers.
  • Train and supervise volunteers and student placements as assigned.
  • Interact with the participants in a professional and polite manner.
  • Ensure the safety and security of program participants at all times.
  • Participate in staff and team meetings, and professional development opportunities as required.
  • Adopt safe work practices, reporting unsafe conditions immediately.
  • Perform other duties as required.

Qualifications and Skills:

  • Postsecondary education in Recreational Therapy, Gerontology, Social Work, and/or a Healthrelated field with gerontology focus and/or working in a community health care environment or an equivalent combination of training and experience.
  • Direct experience working with frail, elderly and/or cognitively impaired seniors and knowledge of their unique needs.
  • Demonstrated experience and skills in working with the frail, elderly and cognitively impaired.
  • Experience with activation techniques with the ability to continually motivate program participants.
  • Must have experience in program planning, and demonstrated dynamic leadership skills in a group setting.
  • Knowledge of a broad range of social, health and community services available in the GTA.
  • Demonstrated organizational and interpersonal skills.
  • Selfdirected, selfmotivated with a positive attitude and be able to work independently and as part of a team.
  • Ability to prioritize, multitask and shift approaches and priorities quickly in response to program participants' needs and interests.
  • Excellent oral and written English skills are required and one or more South Asian language.
  • Sensitivity to people from different socioeconomic backgrounds with an equity lens.
  • Ability to work flexible hours including evenings, weekends and to work from any Indus worksite.
