Position Summary
Reporting to the Manager of Case Planning and Housing Support, the Intensive Case Worker will work as part of a team at 65 Dundas E to provide case management supports based on needs assessments and care plans to 65 Dundas East tenants experiencing serious mental illness and addictions concerns.
The Intensive Case Worker will work with clients to achieve safe, realistic and reasonable goals within a complex health, social and housing environment.
The Intensive Case Worker will work in collaboration with internal and external programs and agencies, with a focus on the long-term success and stability of the client in their housing and in their quality of life.
Duties and Responsibilities
- Support tenants of 65 Dundas E with complex mental health, addiction and physical health needs.
- Conduct needs assessments, develop, and implement comprehensive care plans for tenants and provide case management in the areas of service navigation, managing situational crises and promoting personal wellness.
- Maintain regular and effective communication with other service providers, professionals and partnering agencies.
- Develop and implement plan of care with short and longterm objectives, with ongoing monitoring until tenants no longer require intensive supports or move away from 65 Dundas E.
- Refer and link tenants to community resources and groups that support recovery goals.
- Manage and review the caseload and case plans including by conducting regular reassessments, updating risk levels, updating documentation of progress, next steps, and goal revision by directly consulting with participants.
- Screen and respond to crisis calls initiated by tenants, family members, friends, health care providers, and community agencies.
- Work closely with the Case Planning team as well as the Health and Recovery team, in addition to health care services such as acute care, emergency departments, and other professionals, agencies and groups, to identify problems/gaps and possible solutions for service provision and improvement.
- Actively participate in case planning meetings, including case consultations as part of the Case Planning and Housing team as well as the case consultations with the 65 Dundas E housing provider.
- Assist tenants in navigating the neighbourhood and familiarizing them with key local resources.
- Maintain records, including electronic records, tenant files and case notes and gather data in accordance with project guidelines and requirements appropriate information for statistics and files.
- Attend department team and agency staff meetings.
- Other duties as assigned.
Qualifications
- Postsecondary degree or diploma in social services, social work or other related field or equivalent level of experience (3 4 years of experience as well as trainings, workshops, etc to demonstrate experience and learning).
- Minimum of 3 years related experience in direct service delivery to adults experiencing mental health and/or addictions issues.
- Minimum of 3 years of experience completing assessments, implementing brief interventions and short and long term supports.
- Registration with a professional regulatory organization in Ontario an asset (e.g. RSSW, RSW, etc.).
- Training and experience in Motivational Interviewing, Trauma Informed Care, Interprofessional Multidisciplinary Teams and/or Crisis Intervention Skills an asset.
- Previous experience in working with people experiencing homelessness or who have challenges in obtaining and maintaining housing.
- Demonstrated cultural sensitivity and ability to effectively work with BIPOC and 2SLGBTQ clients.
- Demonstrated experience in supporting individuals with substance use issues from a recovery and harm reduction approach.
- Demonstrated case management and referral skills; and ability to effectively link clients to supportive services.
- Demonstrated experience working within a multidisciplinary team and evidence of an understanding of the importance of partnerships and collaboration.
- Expertise in navigating community mental health and/or community based human services in Toronto.
- Excellent verbal and written communications skills
- Selfdirected and independent, but also experienced in working effectively as a member of a team which includes internal and external stakeholders
- Knowledge of the political, structural, and sociological reasons for homelessness and poverty in the City of Toronto.
- Exceptional organizational skills with ability to meet deadlines and manage multiple priorities.
