We are currently looking to hire a:

Community Supports Manager
1 Full-Time Permanent (1 FTE - 35 hours per week)

Reporting To:
Vice President of Community Services and Strategic Partnerships

Purpose:

The Manager of Community Supports oversees the daily operation of SCHC's Assisted Living and Lawrence East Partnership (LEPP) programs.

These programs are community-based, supporting older adults, individuals with disabilities, caregivers as well as individuals with mental health and addiction challenges to live well at home.

Both the Assisted Living and LEPP programs are located within Toronto Community Housing buildings within the Lawrence Ave/Kingston Rd. neighbourhood of Scarborough and specifically support residents of these buildings.

It is expected that the Manager of Community Supports will help their team to engage clients and community members in a supportive manner to deliver programming reflective of their needs.


Key Areas of Responsibility:

Leadership

  • Understand, interpret, and embody SCHC's mission, vision, and values.
  • Implement SCHC's strategic priorities.
  • Promote person and family centered care in practice with a strong emphasis on an interdisciplinary team approach.
  • Act as a leader in interdisciplinary service provision, emphasizing healthy living specific to client population needs.
  • Support the development of partnerships with stakeholder agencies ensuring integration of services in the community.
  • Promote a welcoming environment consistent with SCHC values for staff, students, volunteers and clients.
  • Facilitate the coordination and integration of services across the organization as well as with external partners and stakeholders.

Team Development and Orientation

  • Oversee the recruitment, retention and performance requirements of team members.
  • Develop and implement individualized plans for professional growth and development, including participation in professional organizations and activities, workshops, seminars, and staff development programs.
  • Monitor team practices ensuring compliance with relevant legislation and applicable standards of practice.
  • Provide supervision, mentorship, orientation, observation, and teaching opportunities, when appropriate, to team members.

General Responsibilities

  • Develop and maintain professional competence.
  • Develop and maintain operational systems where required and ensure the ongoing completion of program administration including:
  • Payroll
  • Collation of monthly statistics, administration of petty cash systems, etc.
  • Development and monitoring of budgets, including variance analysis
  • In conjunction with other staff, identify areas which require additional resources
  • Coordinate and complete reporting requirements in accordance with funder time frames
  • Redesign work flow processes to ensure efficiency and access for clients.
  • Coordinate and support partnerships with multiple and diverse stakeholders.
  • Coordinate and facilitate networking, integration, and collaboration within the community stakeholders.
  • Ensure community priorities are reflected in programs and services offered through the engagement of appropriate stakeholders.
  • Remain current and knowledgeable of issues related to the social determinants of health, community health, homelessness in the GTA, local community trends, etc. particularly in the Scarborough area.
  • Identify trends and issues as they develop and where appropriate develop responsive program initiatives.
  • Develop and implement evaluation processes and identify recommendations and improvements for services.
  • Ensure that community members are regularly consulted in program design and evaluation processes.
  • Coordinate and facilitate networking, integration, and collaboration within the community stakeholders.
  • Ensure community priorities are reflected in programs and services offered.

Resource Development

  • Assesses the resource needs for program areas and identify opportunities for resource development.
  • Develops strategies to secure funding, including:
  • Liaising with funders
  • Drafting funding proposals
  • Developing and maintaining constructive working relationships with existing partners.
