About the Opportunity

The Health Care Social Worker is part of the health care team providing holistic care across various care levels. They focus on the relationship between individuals and their environments to promote psychosocial, emotional, and spiritual well-being. Social Workers emphasize leveraging patients' strengths, coping strategies, problem-solving skills, past accomplishments, and personal resources.

About You

If you have the following qualifications, we want to hear from you:

  • Health-related Bachelor's degree from an accredited university
  • Membership in a health-specific discipline
  • One year of relevant experience in Primary Health Care or community-based settings, including individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles
  • Valid driver's license and daily access to a reliable car for work-related travel
  • Experience in partnership development, stakeholder collaboration, and community development
  • Experience working with individuals with chronic conditions
  • Knowledge of population health, health promotion strategies, and behavior change principles
  • Familiarity with local health services
  • Ability to provide person-centered, family-focused, diversity-relevant care
  • Strong interpersonal, teamwork, and communication skills
  • Flexibility, adaptability, and willingness to innovate
  • Proficiency in program planning, implementation, evaluation, and quality improvement
  • Computer skills including MS Office, email, and internet
  • Certifications in CPR-AED, WHMIS, and proficiency in other languages are advantageous
Hours
  • Full-time position with 75 hours bi-weekly
Compensation and Incentives

$42.89 Hourly
Successful candidates may be eligible for a benefits package that includes health, dental, travel, long-term disability, life insurance coverage, and a defined benefit pension plan.
