About the Opportunity

The Health Care Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the primary to quaternary care continuum. Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.

About You

We would love to hear from you if you have the following:

  • Bachelor's degree in a health-related discipline from an accredited university required
  • Membership with health specific discipline
  • One-year related experience required working in a Primary Health Care setting/community-based team/community setting and experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles; experience working with people with chronic disease in an exercise program or setting.
  • Valid driver's license and daily access to a reliable car for travel for work, required to work at multiple sites and travel between various community locations throughout the catchment area.
  • Preferred experience in partnership development, stakeholder collaboration, and community development
  • Experience working with people living with a chronic condition.
  • Understanding of population health, health promotion strategies, and behavior change principles.
  • In-depth knowledge of local district health authority and community-based services.
  • Demonstrates person-centered / family-focused, diversity relevant approaches to care.
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability, and willingness to change.
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills.
  • Computer skills (MS Office, e-mail, and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Competencies in other languages preferred, French an asset.

  • Permanent full-time position; 75 hours bi-weekly
Compensation and Incentives

$ $42.89 Hourly
