Acclaim Health announces the availability of one (1) Full-Time Care Manager, PSW - 18 month contract position in the Health Services team.

This position is located in the Oakville Office supervising the Burlington PSW team.

As a member of the Health Services team the incumbent is responsible for working in the community providing the supervision, support and training to personal support workers.

The incumbent will work Monday - Friday, 7.5 hours per day with on call rotation with other members of the team.


PRIMARY FUNCTIONS:

  • Identifies, minimizes and manages patient risk in accordance with risk management and incident reporting standards and policies
  • Participates and collaborates with a multidisciplinary care team including staff, patients and caregivers in supporting the patient's needs according to the patient's care plan.
  • Assesses and monitors patient status ensuring that collaborative patientcentred Care Plans are in place and current
  • Provides internal customer service excellence through leading by example utilizing coaching and mentoring to support employees in providing excellence in patient care
  • Under the direction of the Manager, Personal Support, develops and implements quality initiatives aimed at correcting negative variances, addressing gaps, mitigating risks and promoting client safety
  • Works in collaboration with Human Resources in recruitment and orientation of Personal Support Workers.
Effectively manages the performance of direct reports through monitoring and regular performance reviews

  • Effectively manages PSW time off from work, including vacation requests, sick time, and short term LOA requests and records such in Alayacare
  • Ensures that direct reports understand and are aligned to their key accountabilities and perform to the expected level
  • Provides input into the development of the PSW education plan
  • Leads and cofacilitates PSW Team meetings
  • Collaborates with the Manager, Personal Support in the annual review/revision of PSW policies

QUALIFICATIONS & EDUCATION:

- Diploma as Registered Practical Nurse

  • Valid Ontario driver's license, use of vehicle and insurance per Acclaim Health policy.

EXPERIENCE & SKILLS:

  • Minimum 2 to 3 years as a Registered Practical Nurse
  • Demonstrated ability to coach, mentor and lead staff
  • Strong verbal and written communication and interpersonal skills.
  • Effective listening skills.
  • Demonstrated ability in assessment, problemsolving and negotiating skills.
  • Strong planning, coordinating and organizing skills.
  • Demonstrated knowledge and skills in quality/risk management.
  • Proven ability to multitask and meet deadlines.
  • Proficient in Microsoft Word, Excel, Outlook and Alayacare.
  • Knowledge of community resources.
  • Basic knowledge of Acclaim Health and its services.
  • Demonstrated ability to work independently and collaboratively as a team member.
