Reporting to the Director of Health Professions and Collaborative Practice and Psychosocial Practice Head you will

influence and advance Social Work Practice across the organization. The Social Work Practice Lead will provide

leadership and consultation to advance social work practice by adopting and maintaining evidence based practice

and identifying professional practice priorities that improve patient and family outcomes. The Social Work Practice

Lead develops, implements and evaluates the strategic direction for social work practice in collaboration with the

Director of Health Professions and Collaborative Practice, Psychosocial Practice Head and social work management.

The Social Work Practice Lead will role model and embed TBRHSC's patient and family centred care philosophy and

values into all aspects of practice and research.

EMPLOYMENT REQUIREMENTS

  • Unencumbered registration with the Ontario College of Social Workers and Social Service Workers
  • Baccalaureate degree in Social Work with preference given to a Master's degree related to health care
  • Actively involved in professional organizations associated with professional practice (Ontario Association of Social
Workers)

  • Minimum 5 years clinical social work experience
  • 3‐5 years leadership experience
  • Demonstrated record of a strong ethical practice, leadership and motivational abilities
  • Demonstrated ability to influence organization and professional directions with sound organizational awareness
  • Demonstrated knowledge and skill in project management, process improvement, and evaluation
  • Knowledge of adult learning principles and experience in developing, implementing and delivering education
programs

  • Evidence of proficiency in interprofessional collaboration ‐ establishing effective interpersonal relationships,
partnerships, motivating others, consensus‐building, conflict resolution, consultation and coaching/mentoring style of

leadership

  • Knowledge of current professional practice issues and care delivery systems
  • Demonstrated proficiency in communication (verbal and written), group facilitation, problem‐solving, complex
analysis, change management and innovation

  • Understanding and ability to monitor and track data, indicators and produce performance metric reports
  • Demonstrated ability to foster clinical inquiry, educate on research process and develop capacity to conduct
research within social work

  • Participate in research activities and pursue research study that improve patient outcomes.
