The West Elgin Community Health Centre is located in the beautiful rural community of West Lorne, Ontario conveniently situated a half hour outside of London and Chatham.18 Month Contract Employment (37.5 Hours a Week)up to $41.06/hour with competitive benefits and HOOPPRegistered Nurse, Diabetes Education Program - Primary ResponsibilitiesPerform a client assessment, develop a client-centred action plan, implement, evaluate and monitor the plan independently or with the Diabetes Team.
Work in accordance with the CNO standards of practice, policies, procedures and medical directives of the Centre.
Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services.
Participate in a variety of interdisciplinary practice initiatives.
What Do You Bring to the Position?Nursing degree/diploma and additional relevant training.
Member in good standing with the CNO.Canadian Diabetes Educator certification preferred. Flexible to work in West Lorne and occasionally at the Dutton Medical Centre main office.
Strong understanding of various computer programs.
Experience with PS Suite EMR an asset.
Flexibility to work evenings according to program needs.
Verification of full Covid-19 vaccination status or verified medical exemption and vulnerable sector police check required.
