Reporting to the Director of Programs and Operations, the Veterans Transition Worker enhances the dignity and quality of life of Good Shepherd Ministries' clients who served in the Canadian or Allied Forces.

Aftercare and Life Skills support are provided to those who have secured independent accommodation through Good Shepherd Ministries and Veterans Affairs Canada (VAC) "Leave the Streets Behind" Program.


Our values of hospitality, compassion, faith and acceptance are demonstrated by professional collaboration with statutory agencies and other voluntary bodies to provide comprehensive case management services for the clients of Good Shepherd Ministries.


Duties and Responsibilities:

- assist homeless Veterans during the resettlement process and provide support, direction and encouragement
- provide comprehensive After Care services for Veterans moving to independent living from Good Shepherd Ministries in partnership with VAC "Leave the Streets Behind" Program
- co-ordinate case management/counselling services with VAC case workers to provide enhanced supports to Veterans who are housed through Good Shepherd Ministries or VAC, focusing on assisting clients in maintaining long-term accommodation
- liaise with Good Shepherd Ministries Drug and Alcohol Recovery Enrichment (DARE) Program in order to recruit and co-manage Veteran clients; be the lead organizer and facilitator of Veterans Life Skills/Social Skills groups
- provide outreach services to Veterans with complex needs, concurrent disorders, mental health, trauma, which include providing information on drug/alcohol recovery, referrals to appropriate services, crisis intervention, and eviction prevention
- liaise with community agencies that could have contact with Veterans and conduct presentations to community agencies on Good Shepherd Ministries' Veterans initiative
- advocate on behalf of Veterans to improve access to services
- perform any other related tasks that may from time to time be assigned by the Director of Programs and Operations, designate.

Qualifications:

  • Bachelor's degree in social work from a recognized institution.
  • A minimum of two (2) years experience in outreach work, mental health, addiction/housing support, and case management.
  • Experience working with Veterans is an asset.
  • A thorough understanding of homelessness, poverty, health, mental health (PTSD), Addictions, and related issues is required.
  • Solid experience working with the homeless in conducting assessments, case management, crisis intervention, and case coordination is required.
  • Excellent group facilitation skills.
  • Strong motivational counselling skills.
  • Excellent written communication and interpersonal skills to maintain teamwork with the ability to communicate effectively with individuals from diverse backgrounds
  • The ability to work independently and with diverse teams and or different departments.
  • Strong interpersonal and problemsolving skills to maintain teamwork and work professionally with clients, visitors and coworkers.
  • Strong computer skills
  • A valid First Aid, CPR/AED Level C Certificate

Salary:
$28.28-$29.00 per hour
