  • General Description:_
The Sarnia-Lambton Native Friendship Centre (SLNFC) is seeking a dynamic individual to join its team as
Lifelong Care Coordinator.
Under the supervision of the Executive Director and or designate, the Lifelong Care
Coordinator will provide community support services based on the community needs.
The Lifelong Care Coordinator will act as a collaborator to other Sarnia-Lambton Native
Friendship Centre staff in the delivery of culturally appropriate services and events.

The Lifelong Care Coordinator will provide culturally appropriate community support services to urban Indigenous individuals who are elderly, frail, physically disabled, and chronically ill, which promote an integrated response, community participation, independent living, and an improved quality of life.

The program provides transportation, adult day programming and congregate dining, and Indigenous support services.

Duties and Responsibilities:_

  • Ensure the program delivery is congruent with the with the Sarnia-Lambton Native Friendship Centre's mission, vision, and philosophy.
  • Encourages program participants, their caregivers, and staff to participate in cultural and traditional events.
  • Maintain accurate, comprehensive documentation regarding all interactions with program participants. Documentation must in be in accordance with SLNFC standards and adhere to funder requirements.
  • Assist in program and service development to ensure that quality, effective and client
- centered and program participant programs, and services are delivered.

  • Work collaboratively with all team members and partnering agencies to achieve desired excellent service and outcomes.
  • Adhere to all policies governing the Sarnia-Lambton Native Friendship Centre

Qualifications and Experience Required for the Position:_

  • Successful completion of two years of postsecondary education, diploma/degree in Social Work or in a related human services field, with specialized training with Indigenous seniors and vulnerable populations in a longterm care setting.
  • Knowledge of chronic health conditions and gerontologyrelated issues an asset.
  • Knowledge of healthrelated legislation and/or any other policies relevant to program delivery.
  • Strong knowledge and awareness of Indigenous culture and history, with particular emphasis on residential schooling, the impacts of trauma, Indigenous engagement, and community outreach; knowledge of an Indigenous language(s) considered an asset.
  • Knowledge and experience working with urban Indigenous seniors and families in a healing and reconciliation capacity.
  • Understanding of the barriers and issues facing Urban based Indigenous individuals.
  • Ability to deliver programming to improve health conditions, i.e., elder abuse prevention, Diabetes prevention and/or management, chronic health conditions management etc.
  • Ability to engage people to build trust and rapport; effective verbal and listening communication skills; effective crisis intervention skills.
  • Excellent case and file management skills, stress management and time management skills, ability to meet deadlines.
  • Group facilitation, team building and decisionmaking skills; able to problem solve and take initiative.
  • Selfmotivated individual with the ability to work with mínimal supervision in a team
- orientated setting and ensure confidentiality.

  • Ability to effectively work as a team player.
  • Ability to work flexible hours, including evenings and/or weekends.
  • Excellent organizational and time management skills and takes initiative.
  • Excellent verbal and written communication and interpersonal skills.
  • Current certification in Standard First Aid and Level "C" CPR required.
  • Safe Food Handling Course, an asset.
  • Must provide a clear criminal reference check and Vulnerable Sector Clearance.
  • Must provide copy of valid Ontario Driver's License and current and up to date drivers abstract prior to employment.
  • Will be required to provide current copy of government photo identification, emergency contact information.
