The Opportunity
The LiveWell program represents best practice exercise programming for people managing chronic conditions. It was developed and supported by university researchers and YMCAsS. Using a three-phase approach, the program guides participants through progressive exercise strategies, in a client centred
- group setting. The LiveWell program is intended as a resource for community members with stable medical conditions, or multiple co-morbidities, who are wanting to improve their wellbeing. The goal is to help aging adults become and remain active.

This position will facilitate the program at the YMCA of Pictou County.

You will serve as a critical role in growing the program in the region by administering assessments, facilitating classes and knowledge translation.

You will work closely with the local YMCA and Provincial Lead to the successful mandate of the project.

The LiveWell brand represents best practice exercise programs developed by university researchers and YMCAs for people managing chronic disease.

Nova Scotia's version of LiveWell has evolved to three phases of programming where the participants are guided through progressive exercise strategies.


LiveWell programs are intended for community members with stable medical conditions, or multiple co-morbidities, wanting to develop fitness and well-being.

The goal is to help aging adults remain active and fit.

This is a supervised exercise program delivered or supervised by a Clinical Exercise Physiologist who can modify and adapt exercise routines to individual needs.


Responsibilities

  • Work with LiveWell senior program lead, and individual local? local YMCA fitness professionals and program managers, to develop exercise and education programs to support participants managing chronic conditions in three phases of programing: Level disease specific and low functional capacity (e.g., ACCESS/EXCEL, Cardiac Rehab), Level stable to progressive functional capacity (e.g., Active for Life) and Transition to appropriate YMCA general programming.
  • Work with the senior program lead to create a safe system of admitting participants into the program (i.e., risk screening, chronic conditions review, coordinate & administer fitness assessment)
  • With the insight of the senior program lead, systemize and work with regional membership staff, to onboard participants into the appropriate LiveWell program (i.e., financial assistance; membership administration, SDH barrier elimination, etc.)
  • Develop exercise programs for participants and individualize exercise prescriptions in a group setting based on needs using a client focus approach
  • Deliver/manage or oversee exercise sessions
  • Monitor participants' exercise technique and supervise program instructors, assistants and/or students helping to deliver the program
  • Work with senior program lead to manage equipment and local facility needs to deliver the program.
  • Document program information, participant outcomes (fitness testing) participant use, volunteer hours
  • With the assistance of the senior program lead, develop program opportunities as needed (e.g., develop new rehabilitation programs in collaboration with HCPs; new YMCA programs based on need; etc.)
  • Liaise with relevant partners/stakeholders
  • Attend regular LiveWell regional leads meetings and HCP meetings

Qualifications & Skills

  • Bachelor of Kinesiology/Bachelor of Science (Kinesiology) degree (or equivalent)
  • CSEP or ACSM Clinical Exercise Physiologist
  • Experience recognizing, assessing and supporting marginalized populations through a health promotion, social determinant of health (SDH) lens
