Role Description:

We are seeking a motivated and passionate Community Health Worker(s) to join our team.

The Community Health Worker(s) will work to bridge the gap between the healthcare and shelter sector by facilitating health protection, health promotion, and health assessment work across shelters and drop-ins in the GTA.

This position requires someone who is comfortable providing health promotion in a peer capacity and uses their lived experiences and skills to build trusting relationships with clients.

The Community Health Worker will have the opportunity to pursue areas of personal interest and use their skills and talents to contribute to creating health resources and engagement opportunities.


Roles and Responsibilities:

  • Conduct community outreach and education on a range of health topics.
  • Respond to routine emergencies upon request (i.e., encampment evictions, extreme weather conditions, outbreaks etc... ).
  • Support and promote immunization clinics by connecting with clients and assessing the Clients' vaccine eligibility.
  • Promote health in a peer capacity and use experiences and skills to build trusting relationships with clients and provide education and awareness of the available health services.
  • Codevelop training and resources on different health topics.
  • Provide support to different programs across Inner City Health Associates.
  • Facilitate harm reduction support and distribution of harm reduction supplies.
  • Provide support with any emerging and evolving issues.
  • Report on activities and tasks completed weekly by filling out a form.
  • Support our health assessment team by keeping track of their activities to enhance our Data.
  • Attend team meetings based on request and need.

Qualifications:

  • Current and/or previous lived experience of homelessness is an asset.
  • Fluency in English.
  • Fluency in a second language is an asset.
  • Familiarity with the shelter sector in Toronto.
  • Good interpersonal skills and ability to communicate effectively with others
  • Willingness to travel to other shelters and dropin locations as required (travel will be compensated).
  • Consistent attendance record.
  • Openness to learning new skills and knowledge.
  • Willingness to learn and abide by privacy legislation pertaining to personal health information.
  • Ability to work and gain the trust of members from marginalized and diverse populations, including Indigenous, 2SLGTBQ+, newcomers, persons with disabilities and racialized communities.
  • Ability to work independently and as part of a team.
