Copernicus Lodge is a not-for-profit, charitable and ethno-specific organization.

Located in the heart of the Roncesvalles community in the heart of west-end Toronto, we provide a continuum of care in a predominantly Polish, Roman Catholic environment to meet the changing needs of our seniors.

The Lodge is comprised of an Adult Day Program, 200 independent, affordable self-care apartments with a Supportive Housing program, and a 228 bed Long Term Care home.

We are seeking 2 attending physicians to cover 2 units. You will be joining a great group of physicians who cover the home of 228 residents.

Ten units in the home:
6 units of 26 residents and 4 units of 18 residents.

Key Role Duties:

  • Weekly 34 hour visits/unit to the home on specific days to round on residents and cover urgent matters on other units if the issue cannot be managed by the attending physician over the phone
  • On call during week days/evenings for resident issues with the expectation that a visit to the home may be necessary (pronouncements, acute illness): # of calls varies and most issues can be dealt with over the telephone ; nursing communicates via secure conversations or telephone
  • Weekend call from Friday 5pm to Monday 8am; most issues can be dealt with over the phone or by secure conversations, but if deemed clinically necessary a visit to the home is expected Pleasant personality and enjoy working with people, especially seniors
  • Provide handover before and after call for residents who need follow up
  • Attendance at family care conferences which occur 6 weeks after admission, yearly, and as needed
  • Regular thorough documentation in the electronic medical record
  • Regular communication with SDM and regular Goals of Care discussions with families and competent residents

Requirements:

  • Experience/interest in palliative care and/or Care of the Elderly is an asset
  • Continued CME in Care of the Elderly and remaining up to date with current evidence
  • Regular COVID testing as per TPH guidance (done in the home)
  • Proof of up to date COVID Vaccination

Remuneration:

  • Fee for service
  • W010 monthly code, A codes with W special visit codes, K124 for conferences, palliative care codes, on call stipend for weekend call.

  • Covid19 Testing.
