about the role:
The Program Worker will provide determinants of health information to clients and community members and will make appropriate internal and external referrals.
The Program Worker will also provide dedicated support to Women in Need Klinic (WINK), Harm Reduction programming, and the Health Bus.
The Program Worker will be involved supporting new internal and external partner programming.day to day responsibilities:
Direct Service:
- Attends scheduled Health Bus stops as required by the program.
- Ensures the Bus is stocked with supplies and helps to ensure efficient flow of clients while at the Health Bus stops.
- Safely drives the Bus to and from stops.
- Provides support to WINK programming including set up, distribution of food and refreshments, personal hygiene products and harm reduction supplies etc.,
- Provides program information to clients and also provide them with internal and external referrals where necessary
- Attends shifts in STASH dropin, providing Harm Reduction supplies and education/counselling, Naloxone training, and other items stored in dropin to clients as needed. Fulfills STASH specific documentation and tracking requirements
- Communicates effectively while on site with Sherbourne staff and partner agency staff to ensure needs are met and clients are receiving effective and efficient service.
- Documents all client encounters in the EMR system, ensuring documentation is completed according to Timely Documentation policy.
- Maintains supplies in the Harm Reduction Carts and in the Harm Reduction cabinets outside the Centre
- Organizes and maintains the Health Bus supply room & harm reduction equipment throughout the Centre.
- Accompany clients to offsite appointments, as necessary.
- Support with case management of clients in primary care as well as within WINK, Health Bus, and Harm Reduction programming.
Bus Maintenance:
- Organizes routine maintenance for the Bus, aiming to protect the vehicle functionality and with a goal of vehicle longevity.
- Proactively communicates required maintenance to the Health Bus Supervisor
- Takes the Bus to regular scheduled maintenance appointments.
- Liaise with maintenance provider to ensure appointments are scheduled and issues are addressed.
- Escalates safety issues and mechanical issues to the Health Bus Supervisor
General Administration:
- Represents the Integrated Health Services on assigned internal committees, working groups and teams.
- Actively participates in Integrated Health Services program initiatives.
- Provides constructive feedback on the program design and output.
- Other duties as assigned.
