The ideal candidate will be engaged and customer focused individual to take on the role of Bilingual Case Manager. This role is responsible for undertaking the daily management of specific cases that are assigned to them including patient enrolment, nurse assignment and ongoing communication across the Patient Care Team. The Case Manager is responsible for overall management of their patient cases and acts as a single point of contact to their assigned health care providers.


You will assist patients by organizing and scheduling services offered by the patient support program and act as a point person for the manufacturer. You are professional, a strong communicator and courteous.


Accountabilities

  • Perform an in-depth review of multiple data files for select Patient Support Programs (PSP)
  • Oversee the assignment of nurses to patient files for injection services
  • Collaborate with pharma reps and physicians on special initiatives
  • Provide thorough customer service to all patients and customers of the PSP
  • Identify possible adverse events [AE] within data files and prepare an AE Report corresponding to every identified AE within required timelines
  • Fulfil other duties as required.

  • Foster and build relationships with patients and key health care providers;
  • Capable of working independently and able to escalate situations as required
  • Discretion, maturity, flexibility and sound judgement;
  • Ability to work in cross-functional team
  • Computer skills (Word/Excel/CRM/Teams);
  • Ability to manage deadlines;
  • Strong communication skills, both oral and written [English/French]


Qualifications:

  • College degree or equivalent;
  • Fully bilingual French and English. Written and oral.
  • Strong communication and interpersonal skills; professional/courteous/efficient
  • Critical thinking, problem solving abilities and an ability to resolve issues effectively and efficiently;
  • Minimum 1 year of PSP or relevant experience.
  • Experience in the healthcare, pharmaceutical, or insurance industry is an asset;
