We are seeking a highly motivated and skilled Clinical Pharmacist to join our dynamic team. This hybrid full-time position involves a combination of on-site and remote work, offering flexibility while ensuring comprehensive clinical services and effective MedsCheck programs. The ideal candidate will have a passion for patient care, strong clinical knowledge, and excellent communication skills.

Key Responsibilities:

· Provide clinical services to patients, including medication therapy management, chronic disease management, and patient education.

· Conduct comprehensive MedsCheck reviews to ensure patients' medications are optimized and safely managed.

· Collaborate with healthcare providers to develop and implement individualized patient care plans.

· Monitor patient outcomes and provide follow-up consultations to ensure effective treatment and adherence.

· Educate patients on the proper use of medications, potential side effects, and lifestyle modifications for better health outcomes.

· Maintain accurate and up-to-date patient records, ensuring compliance with regulatory standards.

· Participate in continuing education and professional development activities to stay current with the latest clinical practices and advancements.

Qualifications:

· Licensed Pharmacist in [State/Province].

· PharmD degree from an accredited school of pharmacy.

· Minimum of 2 years of clinical pharmacy experience.

· Proficiency in conducting MedsCheck reviews and medication therapy management.

· Strong communication and interpersonal skills.

· Ability to work independently and as part of a team in a hybrid work environment.

· Detail-oriented with excellent organizational skills.

· Proficiency in using pharmacy management software and electronic health records (EHR).
