Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is pleased to partner with various agencies in the Peel Region, to offer services to the local community at the Brampton based Homeless and Addictions Recovery Treatment (HART) Hub.

HART Hubs are community-based care hubs designed to address mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion.

We’re looking for an Addictions Support Worker to join the HART Hub Brampton team

The Addictions Support Worker will conduct comprehensive assessments, provide clinical treatment planning, client-centered brief interventions and case management supports, and facilitate groups as required.

Status: Permanent full-time, 12 hour shifts on a rotational schedule, Monday through Sunday. Shift time is 9am-9pm. Note, this posting is for an active vacancy. Salary: $62,425.03-$73,441.21 annually. (Candidate qualifications will be considered when determining the starting salary). Location: This position is based out of 10 Peel Centre Drive, Brampton office. Due to the nature of this program, this role is on-site and/or community based.

What You Will Do:

• 
  
• Conduct comprehensive standardized addiction assessments, including conducting risk and psychosocial assessments
  
• Provide individual, short-term counselling and case management supports to help support a client’s achievement of their recovery goals.
  
• Facilitate recovery and/or social-recreational groups as needed, including developing new group materials.
  
• Regularly assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate.
  
• Provide education on substance use and wellness to clients and their families/other supports
  
• Refer and link clients to community resources and groups that support recovery goals, including treatment services as appropriate.
  
• Manage a caseload including regular re-assessments, updating risk levels and caseload reviews
  
• Actively participate in staff meetings, team huddles, supervisions, case conferences, trainings, and events as required.
  
• Contact primary care at regular intervals to report clients’ care plan and discharge summary
  
• Work collaboratively with other Addictions Workers to support organizational needs (e.g., volumes across all regions)

What You Need to Join Our Team:

• Post-secondary degree or diploma in relevant discipline
  
• Registration with a regulatory college considered an asset
  
• Minimum 2 years experience in the mental health and addictions sectors, with a focus on substance use, addiction, or concurrent disorders, providing brief interventions and short term supports
